摘  要:目的探究β1肾上腺素受体389位点(rs 1801253)的基因多态性对美托洛尔疗效的影响。方法从中国知网、万方、维普、Embase、SCIE、PubMed等数据库检索从建库至今发表的关于ADRB1389位点基因多态性与美托洛尔有关的文献,通过纳入及排除标准筛选文献,并进行文献质量评价、提取数据,使用RevMan 5.3软件进行定量meta分析。结果最终纳入5篇文献,基因型分布包括:317例Gly389携带者(GG+GC),250例Arg389纯合突变型(CC)。在收缩压控制方面,美托洛尔在CC中疗效优于GG+GC(P=0.01),而突变杂合型GC与野生型GG之间无统计学差异(P=0.77);在舒张压控制方面,CC人群美托洛尔疗效显著优于GG+GC(P=0.0003),而GG与GC之间无统计学差异(P=0.68);在心率控制方面,CC人群美托洛尔疗效同样显著优于GG+GC(P=0.0006),而GC与GG之间无统计学差异(P=0.49)。结论不同的ADRB1389位点基因型对美托洛尔疗效有显著影响,且美托洛尔在CC人群中有更好的疗效,而GG和GC两基因型对美托洛尔疗效的影响无明显差异。Objective Conduct a meta-analysis to investigate the associations betweenβ1-adrenergic receptor(ADRB1)389 locus(rs 1801253)gene polymorphisms and metoprolol treatment outcomes.Methods Search all relevant studies about metoprolol pharmacodynamics and ADRB1389 polymorphisms using the VIP,Wanfang,CNKI,Embase,SCIE and PubMed databases up to the end of today.Meta-analysis was performed by RevMan5.3 following quality assessment and data extraction.Results A total of 5 literatures were adopted,including 317 gene type of GG+GC and 250 gene type of CC.In systolic pressure control,CC was superior to GG+GC(P=0.01).The difference is that there was no statistical distinction between GG and GC(P=0.77).In diastolic blood pressure control,CC was significantly superior to GG+GC(P=0.0003),while no difference watched between GC and GG(P=0.68).In heart rate control,CC was significantly superior to GG+GC(P=0.0006),while no difference watched between GC and GG(P=0.49).Conclusion Different ADRB1389 genotypes have different effects on metoprolol pharmacodynamics,and metoprolol has better efficacy in CC,while GG and GC have no statistical difference in the efficacy of metoprolol.
