中药酒灸辅助治疗小儿肠系膜淋巴结炎(中寒型)的临床研究

摘　　要：目的观察中药酒灸辅助治疗小儿肠系膜淋巴结炎(中寒型)的临床疗效。方法将88例中寒型小儿肠系膜淋巴结炎患儿随机分为治疗组和对照组,每组44例。对照组仅予西医常规治疗,治疗组给予中药酒灸联合西医常规治疗。两组均治疗5 d,观察临床疗效、症状总积分、肠系淋巴结长径和横径、腹痛持续时间、复发率、安全性等情况。结果(1)治疗组、对照组总有效率分别为95.4%和88.6%,组间临床疗效比较,差异有统计学意义(P<0.05);(2)治疗前后组内比较,两组中医证候总积分及肠系膜淋巴结长径、横径差异均有统计学意义(P<0.05);组间治疗后比较,中医证候总积分及肠系膜淋巴结长径、横径差异均有统计学意义(P<0.05);(3)治疗组(23例)腹痛消失时间较对照组(14例)明显缩短,差异有统计学意义(P<0.05);(4)治疗组、对照组复发率分别为8.7%和57.1%;组间复发情况比较,差异有统计学意义(P<0.05);(5)在试验期间,两组所有患儿均未发生尿常规异常、肝肾功能异常及心电图异常等情况。结论中药酒灸辅助治疗中寒型小儿肠系膜淋巴结炎临床疗效显著,可缩短腹痛持续时间,改善症状总积分,缩小肠系膜淋巴结长径、横径值,降低复发率,且操作简单,无不良反应,值得临床进一步推广。Objective To observe the clinical efficacy of alcoholic moxibustion in the treatment of children with mesenteric lymphadenitis(cold type of middle energizer).Methods Eighty-eight cases of mesenteric lymphadenitis in children with cold type of middle energizer were randomly divided into treatment group and control group,44 cases in each group.The control group was given conventional Western medicine treatment only,and the treatment group was given alcoholic moxibustion combined with conventional Western medicine treatment.After 5 days of treatment,the indexes such as the clinical efficacy,total symptom score,length and transverse diameter of intestinal lymph nodes,duration of abdominal pain,recurrence rate and safety were observed.Results(1)The total effective rate of the treatment group and the control group was 95.4% and 88.6% respectively,and there was significant difference between the two groups(P<0.05).(2)There were statistically significant differences in the total score of TCM syndrome and the long diameter and transverse diameter of mesenteric lymph nodes within groups before and after treatment in two groups(P<0.05).After treatment,there were significant differences in the total score of TCM syndrome and the long diameter and transverse diameter of mesenteric lymph nodes between two groups(P<0.05).(3)the disappearance time of abdominal pain in the treatment group(23 cases)was significantly shorter than that in the control group(14 cases).The difference was statistically significant(P<0.05).(4)the recurrence rates in the treatment group and the control group were 8.7% and 57.1%,respectively.There was significant difference in recurrence between the two groups(P<0.05).(5)during the trial,no abnormal urine routine,abnormal liver and kidney function and abnormal ECG occurred in all the children in the two groups.Conclusion The clinical effect of alcoholic moxibustion in the treatment of mesenteric lymphadenitis in children with cold type of middle energizer is obvious.It can shorten the duration of abdominal
