摘　　要：目的:探讨以问题为导向的护理模式对经外周穿刺置入中心静脉导管(PICC)置管患者的影响。方法:采用便利抽签法选择2018年12月1日~2019年12月1日诊治的100例行PICC置管患者为研究对象,采用随机数字表法分为对照组48例和研究组52例,进行单中心随机对照研究,对照组实施常规护理模式,研究组给予以问题为导向的护理模式。比较两组自我护理能力[采用自我护理能力测定量表(ESCA)]、并发症发生率及干预前后负性情绪[采用汉密顿焦虑量表(HAMA)、汉密顿抑郁量表(HAMD)],并评估两组护理质量。结果:研究组ESCA中自护责任感、自护概念、自护技能、将康知识水平得分高于对照组(P<0.01,P<0.05),局部感染、静脉炎等并发症发生率低于对照组(P<0.05);干预后,两组HAMA、HAMD评分低于干预前(P<0.05),且研究组低于对照组(P<0.01);研究组护理人员护理质量得分高于对照组(P<0.01)。结论:以问题为导向的护理模式可有效提高PICC置管患者自我护理能力,减轻其负性情绪,减少术后并发症,并提高护理人员的护理质量。Objective:To explore the effect of problem-oriented nursing model on patients with PICC.Methods:One hundred patients undergoing PICC from December 1,2018 to December 1,2019 were selected as the study subjects by the convenience lottery method.They were divided into the control group of 48 cases and 52 cases of study group.Single-center randomized controlled studies were conducted.The control group was given routine nursing model,and the study group was given problem-oriented nursing model.Self-care ability(ESCA),incidence of complications and negative emotions(HAMA and HAMD)were compared between the two groups,and the quality of nursing was evaluated.Results:The score of self-care responsibility,self-care concept,self-care skill and health knowledge in ESCA was higher in the study group than in the control group(P<0.01,P<0.05),and the incidence of complications such as local infection and phlebitis was lower than that in the control group(P<0.05).After intervention,HAMA and HAMD scores in both groups were lower than those before intervention(P<0.05),and those in the study group were lower than those in the control group(P<0.01).The nursing quality score of nursing staff in the study group was higher than that in the control group(P<0.01).Conclusion:Problem-oriented nursing model can effectively improve self-care ability of patients with PICC,reduce their negative emotions,lower postoperative complications,and promote nursing quality of nursing staff.
