摘 要:目的探讨KARL迭代重建技术联合双低剂量在下肢CTA检查中的应用。方法选取我院疑有下肢动脉阻塞性疾病行下肢CTA检查患者60例,随机分为A、B两组,每组30例,均使用国产联影UCT78064排螺旋CT扫描。A组(常规组)管电压120 kV,使用碘海醇(320 mgI/ml),滤波反投影法(FBP)重建图像。B组(双低组)管电压100 kV,使用威视派克(270 mgI/ml),KARL 5级迭代降噪算法重建技术。比较两组CT容量剂量指数(CTDIvol)、剂量长度乘积(DLP)、碘摄入量;动脉CT值、信噪比(SNR)、对比噪声比(CNR)及图像质量主观评分。结果B组CTDIvol、DLP、SNR、CNR低于A组,B组动脉CT值高于A组,两组之间差异有统计学意义(P<0.05);B组碘摄入量较A组减少15.6%;A、B两组图像主观评分无差异。结论基于KARL迭代重建算法联合双低剂量扫描方法,在满足图像质量诊断的前提下降低患者的辐射剂量和碘对比剂的摄入量,使得“双低”扫描方案得以实现。Objective To discuss the application of KARL simultaneous iterative reconstruction technique with double low doses in the CTA examination of lower limbs.Methods We collected 60 patients in our hospital who were suspected with arterial obstructive diseases of lower limbs and carried out the CTA examination of lower limbs,divided them into Group A and B randomly,with 30 patients in each group.In Group A(routine).We adopted tube voltage of 120 kV,iohexol(320 mgI/ml)and filtered back projection(FBP)to reconstruct the image.In Group B(double low doses),we adopted tube voltage of 100 kV,Visipaque(270 mgI/ml)and KARL 5-level simultaneous iterative reconstruction technique.We compared the CT volume and dose index(CTDIvol),dose-length product(DLP)and iodine intake,artery CT value,signal to noise ratio(SNR),contrast-noise ratio(CNR)and image quality subjective scoring between two groups.Results The CTDIvol,DLP,SNR and CNR of Group B was far lower than those of Group A,the artery CT value of Group B was higher than that of Group,the differences between groups were statistical significance(P<0.05);the iodine intake of Group B was 15.6%,less than that of Group A;the image subjective scores of two groups were free from any difference.Conclusion Based on KARL simultaneous iterative reconstruction technique with the scanning method of double low doses,the radiation dose and iodinated contrast agent intake of the patient is reduced while meeting the requirement on image quality by diagnosis,so as to realize the“double-low”scanning scheme.
