《针灸大成》中合谷穴的主治、配穴规律研究

摘　　要：目的通过研究《针灸大成》中与合谷穴治疗作用相关的条文,总结合谷穴的主治疾病及其配穴规律。方法检索词包括“合谷”“虎口”“含口”“合骨”“四关”,采用电子检索及手工检索相结合的方法检索符合的条文,通过建立数据库和频次分析,总结合谷穴的主治疾病及其配穴规律。结果在《针灸大成》中,关于合谷的条文共有265条,符合纳入标准的条文共246条。合谷穴治疗中医病症出现的频次为292次,主治病症范围包含:中医内科、中医头面五官科、中医妇科、中医外科、中医儿科、中医骨伤科。其中运用最多的学科为:中医内科和中医头面五官科;内科中排前三位是:脑系病症、肢体经络病症、肺系病症;中医头面五官科中眼部疾病运用频次较高。合谷单穴治疗疾病出现的频次是21次,与合谷穴相关的配穴有179个,所有配穴出现频次为838次。其中排前10的配穴为:曲池、足三里、三阴交、百会、颊车、风池、水沟、复溜、行间、中渚。结论合谷穴广泛运用于中医病症的治疗,尤善于治疗内科疾病和头面五官科疾病,合谷穴的腧穴配伍体现了本经配穴法、同名经配穴法、表里经配穴法、上下配穴法、原络配穴法等配伍法则。Objective To sum up the main indications and points combination rules of hégǔpoint by studying the literatures in Compendium of Acupuncture and Moxibustion.Methods The search terms were“hégǔ”“hukou”“hankou”“hegu”“siguan”,and the qualified literatures were collected by the combination of electronic retrieval and manual retrieval.The main indications and points combination rules of hégǔpoint were summed up through establishment of database and frequency analysis.Results In Compendium of Acupuncture and Moxibustion,there were 265 literatures related to hégǔpoint and 246 literatures that met the inclusion criteria.The total frequency of hégǔpoint in the treatment of diseases was 292.Main indications of hégǔpoint involved traditional Chinese medicine of internal medicine,facial features,gynecology,surgery,pediatrics and orthopedics.The most common diseases treated by hégǔpoint were internal medicine diseases and facial features diseases.The top three diseases in traditional Chinese medicine of internal medicine were brain diseases,body meridians diseases and lung diseases.The application frequency of traditional Chinese medicine of ophthalmic diseases in facial features was relatively high.The frequency of hégǔpoint alone in the treatment of diseases was 21 and there were 179 matching acupoints of hégǔpoint,and the total frequency of the matching acupoints was 838.The top 10 matching acupoints were qūchí,zúsānlǐ,sānyīnjiāo,bǎihuì,jiáchē,fēngchí,shuǐgōu,fùliū,xíngjiān,zhōngzhǔ.Conclusion Hégǔpoint was widely used in the treatment of traditional Chinese medicine diseases,especially in internal medicine and facial features.The points combination rules of hégǔpoint involved combination of affected channel,combination of points of namesake channels,superficies-interior points combination,superior-inferior points combination,yuan-source points and collateral-points combination.
