从“诸涩枯涸,干劲皴揭,皆属于燥”探讨银屑病的治疗

摘　　要：银屑病是一种以红斑、鳞屑为主要表现的、易于复发的慢性、炎症性皮肤病,归属于中医“白疕”的范畴。近年来,我国银屑病的发生不断增加,且随着环境、精神及不良生活方式的影响,银屑病的复发率逐年升高。现代医学以免疫抑制、抗感染、抗增生、生物调节及皮肤修复等治疗为主,但不良反应却难以避免。中医学运用整体观与辨证论治相结合,对本病进行了更深入的研究。因此,基于“诸涩枯涸,干劲皴揭,皆属于燥”理论,结合温病学卫气营血辨证与银屑病诊治的密切关系:系统论述寻常型银屑病应从燥论治,根据“留一分津液,便有一分生机”治疗原则,提出寻常型银屑病血虚血燥证、血热血燥证、血瘀血燥证皆以润燥止痒为基本治则。Psoriasis is a chronic and inflammatory skin disease with erythema and scale as its main manifestations and easy to relapse.Traditional Chinese medicine belongs to the category of“White Scar”.In recent years,the occurrence of psoriasis in China has been increasing continuously,and with the influence of environment,spirit and bad lifestyle,the recurrence rate of psoriasis has increased year by year.Modern medicine focuses on immunosuppression,anti-infection,anti-proliferation,biological regulation and skin repair,but adverse reactions are hard to avoid.Traditional Chinese medicine has made a more in-depth study of this disease by combining the holistic view with syndrome differentiation and treatment.Therefore,based on the theory that“all astringent and dry wine and chapped energy belong to dryness”and combined with the close relationship between the defense of Qi,nourishing blood and the diagnosis and treatment of psoriasis in the science of warm diseases,this paper systematically discusses that psoriasis vulgaris should be treated from dryness.According to the therapeutic principle of“keeping body fluid,there is vitality”,it puts forward that the basic therapeutic principles of psoriasis vulgaris blood deficiency and blood dryness syndrome,blood and blood dryness syndrome and blood stasis dryness syndrome are moistening dryness and relieving itching.
