摘　　要：目的研究Toll样受体4(TLR4)在临产前后孕妇外周血中的表达及与下游相关信号通路激活的关系。方法收集上海中医药大学附属普陀医院2018年3月—2019年8月收治住院的孕妇外周血及子宫肌组织,根据是否临产分为足月临产组(21例)和足月未临产组(20例)。采用流式细胞术、酶联免疫吸附法检测孕妇外周血TLR4的表达,RT-PCR法检测子宫肌组织中TLR4、收缩相关蛋白及核因子(NF)-κB、MAPK家族JNK、ERK、P38 mRNA表达并进行相关性分析。结果两组孕妇的年龄、孕龄、体重指数比较,差异无统计学意义(P>0.05)。两组孕妇的外周血中性粒细胞、单核细胞及血清TLR4表达水平比较,差异有统计学意义(P<0.05)。两组孕妇子宫肌组织中TLR4、CX43及OTR比较,差异有统计学意义(P<0.05)。两组孕妇NF-κB、ERK、P38、JNK mRNA水平比较,差异有统计学意义(P<0.05)。TLR4与收缩相关蛋白CX43、OTR和信号通路JNK、ERK、P38、NF-κB呈正相关(R^2=0.619、0.416、0.438、0.426、0.55、0.332,均P<0.01)。JNK、ERK、P38、NF-κB与收缩相关蛋白CX43呈正相关(R^2=0.548、0.393、0.702、0.600,均P<0.01);与OTR呈正相关(R^2=0.308、0.608、0.532、0.514,均P<0.01)。结论TLR4可能通过激活下游NF-κB/MAPK通路,促进收缩相关蛋白的表达,促进子宫收缩,在分娩启动中发挥重要作用。Objective To analyze the expression of Toll-like receptor 4(TLR4)in maternal peripheral blood before and after parturition and its relationship with activation of downstream signaling pathways.Methods The peripheral blood and uterine muscle tissue of pregnant women admitted to Putuo Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from March 2018 to August 2019 were collected and divided into term labor group(21 cases)and term non labor group(20 cases)according to whether or not labor.The expression of TLR4 was detected by flow cytometry and enzyme-linked immunosorbent assay.RT-PCR was used to detect the expression of TLR4,contraction related protein,nuclear factor(NF)-κB,JNK,ERK and P38 mRNA in uterine muscle,and their relationships were analyzed.Results There was no significant difference in age,gestational age and body mass index between two groups(P>0.05).There were significant differences in the expression levels of peripheral blood neutrophils,monocytes and serum TLR4 between two groups(P<0.05).There were significant differences in TLR4,CX43 and OTR between two groups(P<0.05).There were significant differences in the mRNA levels of NF-κB,ERK,P38 and JNK between two groups(P<0.05).TLR4 was positively correlated with CX43,OTR,JNK,ERK,P38 and NF-κB(R^2=0.619,0.416,0.438,0.426,0.55,0.332,all P<0.01).JNK,ERK,P38 and NF-κB were positively correlated with CX43(R^2=0.548,0.393,0.702,0.600,all P<0.01),JNK,ERK,P38 and NF-κB were positively correlated with OTR(R^2=0.308,0.608,0.532,0.514,all P<0.01).Conclusion TLR4 may play an important role in labor initiation by activating downstream NF-κB/MAPK pathway,promoting the expression of contraction related proteins and promoting uterine contraction.
