摘　　要：目的探讨超声小探头(MPS)在上消化道隆起性病变中的应用价值及体会。方法选取我院2018年1月~2019年12月382例行超声胃镜检查的患者,记录其超声胃镜检查结果,对患者术前超声及术后病理进行对照分析,观察病变大小、起源层次、性质及超声胃镜准确性的影响。结果本研究382例患者经小探头超声内镜检查发现其中病变位于食管156例,胃203例,十二指肠23例。按照病变类型进行分类,其中平滑肌瘤88例,间质瘤124例,囊肿33例,脂肪瘤19例,血管瘤18例,淋巴瘤1例,异位胰腺22例,外压病变16例,息肉15例,十二指肠副乳头4例,早期癌42例。其中243例患者均经EMR、ESD或手术病理证实,与病理诊断符合率达85.60%。结论小探头超声内镜检查能够显示上消化道隆起病变的起源层次及其与邻近组织结构的关系,并能提示隆起性病变的性质,为进一步EMR、ESD或手术治疗提供有力的支持,对消化道肿瘤的T分期具有较高的诊断价值。Objective To investigate the application value and experience of miniprobe sonography(MPS)in protuberant lesions in upper digestive tract.Methods A total of 382 patients who were treated with ultrasonic gastroscopy in our hospital from January 2018 to December 2019 were selected.Their ultrasonic gastroscopy results were recorded.Preoperative ultrasound and postoperative pathology of the patients were compared and analyzed.The size,origin level,nature of protuberances and the accuracy of ultrasonic gastroscopy were observed.Results In 382 patients examined by MPS,the lesions were found in esophagus of 156 patients,stomach of 203 patients and duodenum of 23 patients.According to types of the lesions,there were 88 cases of leiomyoma,124 cases of stromal tumor,33 cases of cyst,19 cases of lipoma,18 cases of hemangioma,1 case of lymphoma,22 cases of ectopic pancreas,16 cases of external pressure lesion,15 cases of polyp,4 cases of duodenal papillary carcinoma,and 42 cases of early cancer.Among them,243 patients were confirmed by EMR,ESD or operation pathology,and the coincidence rate with pathological diagnosis reached 85.60%.Conclusion MPS can reveal the origin level of protuberant lesions in upper digestive tract and their relationship with adjacent tissue structures,and can indicate the nature of the lesions,providing strong support for further EMR,ESD or surgical treatment,and having high diagnostic value for T-stage of digestive tract tumors.
