甲状腺切除手术应用瑞芬太尼联合异丙酚的效果分析与研究

摘　　要：目的分析甲状腺切除手术应用瑞芬太尼联合异丙酚的效果。方法选取2015年1月~2019年12月行甲状腺切除手术患者500例,采用随机数字表法将其分为两组,对照组采用常规麻醉措施,术中持续泵注异丙酚,研究组术中持续静脉输注瑞芬太尼联合异丙酚。比较两组患者各阶段血流动力学指标变化情况、麻醉恢复时间、术后不良反应发生率。结果研究组术前、术中、拔管后的平均动脉压(MAP)、心率(HP)变化程度低于对照组,差异有统计学意义(P<0.05);研究组自主呼吸恢复时间、清醒时间、拔管时间均短于对照组,差异有统计学意义(P<0.05);研究组术后不良反应总发生率低于对照组,差异有统计学意义(P<0.05)。结论甲状腺切除手术应用瑞芬太尼联合异丙酚,能够减少血流动力学改变,缩短麻醉恢复时间,减少术后不良反应发生率,具有较高应用价值的麻醉措施,值得推广应用。Objective To analyze the efficacy of remifentanil combined with propofol in thyroidectomy.Methods From January 2015 to December 2019,500 patients treated with thyroidectomy were randomly divided into the control group and the study group according to the random number table method.The control group was given conventional anesthesia and propofol was continuously pumped during operation,while the study group was intravenously infused remifentanil combined with propofol continuously during operation.The changes of hemodynamic indexes,recovery time of anesthesia and incidences of postoperative adverse reactions(ADRs)were compared between the two groups of patients.Results The changes of the mean arterial pressure(MAP)and heart rate(HP)in the study group were lower than those in the control group before,during and after operation,with statistically significant differences(P<0.05).The recovery time of spontaneous breathing,waking time and extubation time in the study group were all shorter than those in the control group,with statistically significant differences(P<0.05).The total incidence of ADRs in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Remifentanil combined with propofol in thyroidectomy reduce hemodynamic changes,shorten the recovery time of anesthesia and reduce the incidence of postoperative ADRs.Therefore,it is an anesthetic measure with high application value and is worth promoting.
