西药联合穴位埋线对高血压合并OSAS患者血压睡眠呼吸指标IMT血清sCD40L及IL-6水平的影响

摘　　要：目的探讨西药联合穴位埋线治疗对高血压合并阻塞性睡眠呼吸暂停综合征患者血压、睡眠呼吸指标、颈动脉内膜中层厚度、血清可溶性CD40配体、白细胞介素-6水平及生活质量的影响.方法将90例高血压合并阻塞性睡眠呼吸暂停综合征患者按随机数字表法分为研究组与对照组,每组45例,对照组予以常规西医治疗,研究组在对照组基础上联合穴位埋线治疗,连续治疗2个月.治疗前后检测两组血压(收缩压、舒张压)、睡眠呼吸指标(夜间最低血氧饱和度、夜间平均血氧饱和度、最长呼吸暂停时间、睡眠呼吸暂停低通气指数)、颈动脉内膜中层厚度、血清可溶性CD40配体、白细胞介素-6水平变化,采用睡眠呼吸暂停生活质量指数评定生活质量.结果治疗后研究组收缩压、舒张压水平显著低于对照组(P<0.01),睡眠呼吸各项指标改善显著优于对照组(P<0.01),颈动脉内膜中层厚度显著小于对照组(P<0.01);血清可溶性CD40配体、白细胞介素-6水平低于对照组(P<0.01),睡眠呼吸暂停生活质量指数评分显著高于对照组(P<0.01).结论西药联合穴位埋线治疗高血压合并阻塞性睡眠呼吸暂停综合征患者能有效调控血压及睡眠呼吸指标,缩小颈动脉内膜中层厚度,下调血清可溶性CD40配体、白细胞介素-6水平,提高生活质量.Objective To investigate effect of W estern medicine combined with acupoint embedding on blood pressure.sleep respiration index.IMT,serum sCD40L,IL.-6 level and quality of life in patients with hypertension and OSAS.Methods 90 patients with hypertension and OSAS were divided into a re-search group and a control group according to the random number table method,45 cases in each group..The control group was treated with conventional W estern medicine,and the research group was combined with acupoint embedding therapy on the basis of the control group for 2 months of continuous treatment.Before and after treatment.blood pressure(SBP,DBP),sleep respiration index(LSpO2,MSpO2.maxi-mum apnea time,sleep apnea hypopnea index),IMT,serum sCD40L,and interleukin 6 levels were detec-ted.SAQLI was used to assess the quality of life.Results After treatment,the levels of SBP and DBP in the study group were significantly lower than those in the control group(P<0.01).The improvement of sleep breathing indicators was significantly better than that in the control group(P<0.01).IMT was sig-nificantly less than that in the control group(P<0.01);serum sCD40L,and interleukin-6 levels were lower than the control group(P<0.01),SAQLI score was significantly higher than the control group(P<0.01).Conclusions Western medicine combined with acupoint embedding therapy in patients with hypertension and OSAS can effectively regulate blood pressure and sleep breathing indexes,reduce IMT,decrease serum sCD40L and IL-6 levels,and improve quality of life.
