颅脑外伤致额叶损伤严重程度与精神障碍及预后的相关性

摘　　要：目的探讨颅脑外伤致额叶损伤严重程度与精神障碍、预后的相关性.方法将90例颅脑外伤致额叶损伤患者按格拉斯哥昏迷指数评分分为重度组(30例)、中度组(30例)和轻度组(30例),采用格拉斯哥昏迷指数、汉密顿焦虑量表、汉密顿抑郁量表及格拉斯哥结局评定疾病严重程度、精神障碍及预后,并分析疾病严重程度与精神障碍及预后的关系.结果重度组汉密顿焦虑量表、汉密顿抑郁量表评分显著高于中度组和轻度组(P<0.05),中度组显著高于轻度组(P<0.05);重度组格拉斯哥结局评分显著低于中度组和轻度组(P<0.05),中度组显著低于轻度组(P<0.05).不同严重程度患者格拉斯哥昏迷指数评分与汉密顿焦虑量表、汉密顿抑郁量表评分呈显著负相关(P<0.05或0.01),与格拉斯哥结局评分呈显著正相关(P<0.05或0.01).结论颅脑外伤致额叶损伤严重程度与精神障碍、预后有密切关系.Objective To explore the correlatiop between the severity of frontal lobe injury caused by craniocerebral trauma,mental disorder and prognosis.Methods 90 patients with frontal lobe injury caused by craniocerebral trauma were selected as the research objects.According to GCS score,they were divided into severe group(n=30),moderate group(1=30)and mild group(n=30).GCS,HAMA,HAMD and GOS were used to assess disease severity,mental disorders and prognosis,and analyze the re-lationship between disease severity and mental disorders and prognosis.Results The scores of HAMA and HAMD in the severe group were significantly higher than those in the moderate group and mild group(P<0.05),and the moderate group was significantly higher than the mild group(P<0.05).The score of GOS in the severe group was significantly lower than that in the moderate group and mild group(P<0.05),and the moderate group was significantly lower than the mild group(P<0.05).There was a sig-nificant negative correlation between GCS and HAMA and HAMD scores in patients of different severity(P<0.05 or0.01),and a significant positive correlation with GOS(P<0.05 or 0.01).Conclusions The severity of frontal lobe injury caused by craniocerebral trauma is closely related to mental disorders and prognosis.
