基于FTS理念的临床护理路径对全髋关节置换术后患者功能恢复的影响

摘　　要：目的探讨基于快速康复外科理念的临床护理路径对全髋关节置换术后患者功能恢复的影响.方法将2016年11月—2017年9月住院的42全髋关节置换手术例患者设为对照组,2017年10月—2018年8月住院的42例全髋关节置换手术患者设为观察组.对照组给予常规护理,观察组给予基于快速康复外科理念的临床护理路径干预.比较两组患者拔管时间、下床活动时间、住院时间及并发症发生率,于术前及术后1个月末采用髋关节功能评分系统评定两组患者髋关节功能.结果观察组拔管时间及下床活动时间均显著早于对照组(P<0.01),住院时间显著短于对照组(P<0.01),并发症发生率显著低于对照组(P<0.05).术后1个月末两组髋关节功能评分系统评分均较手术前显著升高(P<0.01),观察组较对照组升高更显著(P<0.01).结论基于快速康复外科理念的临床护理路径能显著加快全髋关节置换术后患者的功能恢复,降低并发症发生率,值得推广应用.Objective To investigate the effect of clinical nursing pathway based on fast track surgery(FTS)concept on postoperative recovery of function in patients with total hip arthroplasty.Methods 42 patients hospitalized from November 2016 to September 2017 were set as the control group,and the 42 pa-tients hospitalized from October 2017 to August 2018 were set as the observation group.The control group was given routine care,and the observation group was given F TS based clinical nursing path inter-vention.The extubation time,off-bed activity time,hospital stays and incidence of complications of the two groups were compared.The hip joint function score system was used to evaluate the hip joint function of the two groups of patients before and at the end of 1 month after surgery.Results Compared with con-trol group,the extubation time and off bed activity time in observation group were earlier(P<0.01),the hospital stays in observation group were shorter(P<0.01),the total incidence of complications in obser-vation group was lower(P<0.01),and the Harris score in observation group was higher(P<0.01),the observation group increased more significantly than the control group(P<0.01).Conclusions Clinical nursing pathway based on FTS concept for patients with total hip arthroplasty can accelerate rehabilita-tion,reduce the incidence of complications,and is worthy of popularization and application.[Keywords]
