虹膜炎并发白内障瞳孔粘连手术治疗临床研究

摘　　要：目的研究虹膜炎并发白内障瞳孔粘连手术治疗临床效果。方法方便选取2015年9月—2019年9月该院收治的的80例虹膜炎并发白内障瞳孔粘连患者展开研究,以数字表法为依据将其划分为对照组(40例)与观察组(40例)。行瞳孔钝性分离扩张术+超声乳化人工晶状体植入术的是对照组,行瞳孔缘环形切开术+超声乳化人工晶状体植入术的是观察组,观察两组的治疗效果。结果术后观察两组患者的视力改善情况,观察组视力<0.3的例数明显比对照组的少(χ2=7.912,P=0.005<0.05);观察组视力在0.3~0.6左右的视力与对照组差异无统计学意义(χ2=0.061,P=0.805>0.05);观察组视力>0.6的例数明显比对照组多(χ2=6.545,P=0.011<0.05);术后观察组患者形状规则,有光反射的例数明显比对照组多,观察组患者出现形状不规则,无光反射的例数明显比对照组少(χ2=5.013,P=0.025<0.05);术后观察组患者7.5%的术后并发症率相较于对照组30.0%的术后并发症率明显更低(χ2=6.646,P=0.010<0.05)。结论治疗虹膜炎并发白内障采用瞳孔缘环形切开法联合超声乳化人工晶状体植入术效果较为理想。Objective To study the clinical effect of surgical treatment of iritis complicated with cataract and pupil adhesion.Methods A study was conducted on 80 patients with iritis complicated with cataract and pupillary adhesions convenient selected in the hospital from September 2015 to September 2019.Based on the number table method,they were divided into a control group(40 cases)and an observation group(40 cases).Blunt pupil dilation+phacoemulsification and intraocular lens implantation were performed in the control group,and the pupil rim circular incision+phacoemulsification and intraocular lens implantation was performed in the observation group.The treatment effects of the two groups were observed.Results The visual acuity improvement of the two groups of patients was observed after operation.The number of people in the observation group with visual acuity<0.3 was significantly less than that in the control group(χ2=7.912,P=0.005<0.05);the visual acuity of the observation group was around 0.3-0.6,there was no statistically significant difference between the visual acuity and the control group(χ2=0.061,P=0.805>0.05);the observation group had significantly more people with visual acuity>0.6 than the control group(χ2=6.545,P=0.011<0.05);After the operation,the observation group had regular shape,the observation group had more people with light reflection than the control group,the observation group had irregular shape,and the number of people without light reflection was significantly less than the control group(χ2=5.013,P=0.025<0.05);7.5%of postoperative complications rate in the observation group were significantly lower than that of 30.0%in the control group(χ2=6.646,P=0.010<0.05).Conclusion In the treatment of iritis complicated with cataract,the effect of circular pupillary incision combined with phacoemulsification and intraocular lens implantation is relatively satisfactory.
