摘　　要：目的:探讨房缺封堵伞在高危卵圆孔封堵术中的可行性和安全性。方法:回顾性分析西京医院心脏外科2016-01~2019-01诊断符合高危卵圆孔患者78例,均采用房缺封堵伞经导管介入治疗,分别于术后1、3、6、12个月复查经胸超声、心电图、经颅超声造影及食道超声,评价术后并发症发生情况。结果:78例患者中77例手术成功,成功率为98.9%。2例出现脱伞(2.5%)、6例房颤(7.6%)、1例血管入路并发症(1.3%)。术后12个月复查经颅超声造影,2例(2.6%)出现少-中量介入术后右向左分流。结论:选用房缺封堵伞封堵高危卵圆孔,术后并发症发生率较低,尤其是中期介入术后右向左分流疗效较好,可以安全用于临床。Objective:To investigate the feasibility and safety of umbrella type atrial septal occluder in high-risk patent foramen ovale.Methods:Seventy-eight cases with high-risk patent foramen ovale from January 2016 to January 2019 in department of cardiac surgery of Xijing hospital were analyzed retrospectively,all of them were treated by transcatheter catheter with atrial occlusion,and then reexamined by transthoracic echocardiography,electrocardiogram,control-enhanced-transcranial Doppler,transesophageal echocardiography at 1,3,6 and 12 months after surgery to evaluate postoperative complications.Results:Among the 78 cases,the operation was successful in 77 cases,and the success rate was 98.9%.Two cases(2.5%)were occurred umbrella lost,6 cases(7.6%)were occurred atrial fibrillation and 1 patient(1.3%)were occurred vascular complications.After 12 months of follow-up,2 cases(2.6%)showed right to left shunt after least-medium intervention.Conclusion:The high-risk patent foramen ovale is blocked by umbrella type atrial septal occluder,the incidence of postoperative complications is low,especially the right to left shunt after the mid-stage intervention,and the curative effect is better,which can be used in clinical practice.
