血清IgG4与IgG4/IgG以及相关血清学指标在诊断IgG4相关性疾病及其他自身免疫性疾病中的临床价值分析

摘　　要：目的:探究血清免疫球蛋白G4(IgG4)、IgG4/IgG及相关血清学指标对IgG4相关性疾病(IgG4-RD)及其他自身免疫性疾病(AID)的诊断价值。方法:将2016年10月—2018年4月在我院接受治疗的7例IgG4-RD患者及184例其他AID患者作为研究对象,并随机选取同时期在我院体检的正常人191名作为健康对照,所有研究对象均进行血清IgG、IgG4水平检测,并计算IgG4/IgG数值,观察结果。结果:IgG4-RD患者血清IgG、IgG4、IgE水平及IgG4/IgG数值均高于健康体检者与其他AID患者,差异有统计学意义(P<0.05);其他AID患者与健康体检者血清IgG、IgG4水平及IgG4/IgG数值比较无显著差异(P>0.05);其他AID患者血清IgE水平高于健康体检者,差异显著(P<0.05);治疗后,IgG4-RD患者血清IgG4水平较治疗前明显降低,差异显著(P<0.05)。结论:血清IgG4水平升高可作为IgG4-RD的诊断及疗效评价的指标之一。Objective To explore the value of serum immunoglobulin G4(IgG4),IgG4/IgG and serological indicators on diagnosis of IgG4-related disease(IgG4-RD)and other autoimmune diseases(AID).Methods 7 IgG4-RD patients and 184 other AID patients(2016.10~2018.4)in our hospital were chosen as research subjects.Over the same time period,191 healthy checkups were chosen as healthy controls.Serum IgG and IgG4 levels were measured;IgG4/IgG was accounted.Results Serum IgG,IgG4,IgE and IgG4/IgG values in IgG4-RD patients were significantly higher than healthy checkups and other AID patients(P﹤0.05);serum IgG,IgG4 and IgG4/IgG values between other AID patients and healthy checkups were not significantly different(P﹥0.05);IgE levels in other AID patients were significantly higher than healthy checkups(P﹤0.05);after treatment,serum IgG4 levels in IgG4-RD patients were remarkably lower than before(P﹤0.05).Conclusion Elevated IgG4 levels can be considered as an indicator for clinical diagnosis and efficacy assessment of IgG4-RD.
