摘　　要：目的:比较全髋关节置换术采用自体血回输与异体血输注的输血量及成本差异。方法:将120例患者按随机数字表法均分为自体血回输组和异体血输注组,经过剔除后,自体血回输组实际入组37例,异体血输注组实际入组35例。观察各组患者术中出血量、术中自体血回输量或/和异体血输注量、术后异体血输注量、手术时间、术后1h Hct、自体血回收成本、异体血输注成本。结果:两组患者术前红细胞压积(Hct)、术中失血量、手术时间、术后输血率、术后输血量及术后1h Hct差异均无统计学意义(P均>0.05)。与异体血输注组相比,自体血回输组术中输血率极低且这些患者的输血量较少(P均<0.01)。自体血输注组术中平均花费约为1397.3元,异体血输注组输血费用为2890.4元,自体血输注组费用明显更低。结论:自体血回输能够明显减少患者的医疗费用,费效比较高,应大力提倡。Objective To compare the difference in blood transfusion volume and cost between autotransfusion and allogeneic blood transfusion in total hip arthroplasty.Methods According to random number table,120 patients were equally divided into autotransfusion group and allogeneic blood transfusion group.After exclusion,37 patients were enrolled in autotransfusion group,and 35 patients were enrolled in allogeneic blood transfusion group.The intraoperative blood loss,intraoperative autologous blood transfusion or/and allogeneic blood transfusion volume,postoperative allogeneic blood transfusion volume,operation time,hematocrit(Hct)at 1 hour after operation,autotransfusion cost,and allogeneic blood cost were observed.Results There were no significant differences in preoperative Hct,intraoperative blood loss,operation time,postoperative blood transfusion rate,postoperative blood transfusion volume and Hct at 1 hour after operation(P>0.05).Compared with the allogeneic blood transfusion group,the autotransfusion group had a much lower blood transfusion rate and the allogeneic blood volume of these patients was lower(P<0.01).The average cost in the autotransfusion group was about 1397.3 Yuan,and it was at least 2890.4 Yuan in the allogeneic blood transfusion group.Therefore,the transfusion-related costs in the autotransfusion group was significantly lower than that in the allogeneic blood transfusion group.Conclusion Compared with allogeneic blood transfusion,autologous blood transfusion,which has higher cost-effectiveness ratio,can significantly reduce the medical expenses of patients.It should be strongly advocated.
