推拿联合冲击波治疗桡骨茎突狭窄性腱鞘炎的临床研究

摘　　要：目的比较推拿联合冲击波治疗仪和单用冲击波疗法对桡骨茎突狭窄性腱鞘炎的疼痛缓解及腕关节功能改善情况。方法招募34名桡骨茎突狭窄性腱鞘炎受试者随机分成综合治疗组(16例)和单一治疗组(18例),综合治疗组:推拿联合冲击波治疗,单一治疗组:单用冲击波治疗,分别治疗2周。结局评价为疼痛、腕关节功能评估(NRS量表、Cooney量表),评估时间点在第一周治疗结束后(P1),第二周治疗结束后(P2)。结果P1:两组NRS评分较入组时均有所降低(P<0.05),综合治疗组的NRS评分低于单一治疗组(P<0.05);P2:综合治疗组NRS评分低于P1及对照组(P<0.05);单一治疗组的NRS评分较P1降低,差异具有统计学意义(P<0.05)。2组Cooney腕关节功能评分均较入组时提高(P<0.05),且综合治疗组的改善程度较单一治疗组更为显著(P<0.05)。结论推拿联合冲击波疗法或单用冲击波疗法都能改善桡骨茎突狭窄性腱鞘炎的疼痛及功能受限情况,但推拿联合冲击波治疗的临床疗效较单用冲击波更显著。Objective To compare the effect of Tuina combined with shock wave therapy and shock wave monotherapy on pain relief and wrist function improvement of stenosing tenosynovitis of styloid process of radius.Methods 34 patients with stenosing tenosynovitis of styloid process of radius were randomly divided into two groups:comprehensive treatment group(16 cases)and single treatment group(18 cases).The outcome was evaluated as pain and wrist function(NRS and Cooney scales).The evaluation time was at the end of the first week(P1)and the second week(P2).Results The NRS score of the two groups was lower than that of the single treatment group(P<0.05),the NRS score of the combined treatment group was lower than that of the single treatment group(P<0.05),the NRS score of the combined treatment group was lower than that of the P1 and the control group(P<0.05),and the NRS score of the single treatment group was lower than that of the P1,the difference was statistically significant(P<0.05).The Cooney wrist function score of the two groups was higher than that of the control group(P<0.05),and the improvement of the comprehensive treatment group was more significant than that of the single treatment group(P<0.05).Conclusion Tuina combined with shockwave therapy or shockwave therapy alone can improve the pain and limited function of stenosing tenosynovitis of styloid process of radius,but the clinical effect of Tuina combined with shockwave therapy is more significant than shockwave therapy alone.
