宫腔纱条填塞术与Bakri球囊填塞术应用于剖宫产产后出血的效果比较

摘　　要：目的对比分析宫腔纱条填塞术与Bakri球囊填塞术应用于剖宫产产后出血的效果。方法选取如皋市人民医院2018年3月至2019年10月收治的148例剖宫产产后出血产妇为研究对象,按照随机数字表法分为观察组和对照组,各74例。对照组给予宫腔纱条填塞术,观察组给予Bakri球囊填塞术。对比两组手术情况、住院时间、止血成功率、剖宫产前后血流动力学水平及术后并发症发生率情况。结果观察组手术时间、术中出血量、术后24 h出血量及输血量均低于对照组(P<0.05);观察组止血成功率高于对照组(P<0.05);观察组产后2 h的心率低于对照组,收缩压、舒张压及血氧饱和度高于对照组(P<0.05);观察组术后总并发症发生率低于对照组(P<0.05)。结论应用Bakri球囊填塞术治疗剖宫产产后出血的止血效果优于宫腔纱条填塞术,可改善手术情况及血流动力学水平,降低术后并发症发生率,值得临床推广。Objective To compare and analyze the effect of uterine gauze packing and Bakri balloon packing applied to postpartum hemorrhage in cesarean section.Methods Totally 148 patients with postpartum hemorrhage after cesarean section admitted to Rugao People's Hospital from March 2018 to October 2019 were selected as the study subjects,and were divided into observation group and control group,with 74 cases each.The control group was given uterine gauze packing and the observation group was given Bakri balloon packing.The operation,length of hospital stay,hemostatic success rate,hemodynamic level before and after cesarean section and incidence of postoperative complications were compared between the two groups.Results The operation time,intraoperative blood loss,24 h postoperative blood loss and blood transfusion of the observation group were all lower than that of the control group(P<0.05);The hemostatic success rate of the observation group was higher than that of the control group(P<0.05);The heart rate of the observation group 2 hours after delivery was lower than that of the control group,and the systolic pressure,diastolic pressure and blood oxygen saturation were high compared with the control group(P<0.05);the total postoperative complication rate of the observation group was lower than that of the control group(P<0.05).Conclusion Bakri balloon tamponade is superior to uterine gauze tamponade in hemorrhage after cesarean section.It can improve the surgical condition and hemodynamic level,reduce the incidence of postoperative complications,and is worthy of promotion.
