摘 要:目的:评价前列爽对前列腺增生的防治作用并探讨其可能的作用机制。方法:以雄性小鼠和大鼠复制前列腺增生模型,灌胃前列爽(小鼠2.0、4.0、8.0 g/kg,大鼠1.5、3、6 g/kg),取前列腺称重计算其指数。评价其对前列腺指数、腺上皮面积、前列腺病理的影响;另取连续造模5 w,连续给药5 w大鼠,末次给药半小时后,测定基础膀胱内压,静脉注射盐酸去氧肾上腺素2.0 mg/kg造模,观察静脉注射药物后膀胱内压的变化,取前列腺称重计算其指数。结果:小鼠在注射丙酸睾丸酮3 w后,与正常对照组比较,模型对照组前列腺指数显著升高(P<0.01);与模型对照组比较,前列爽8.0、4.0、2.0 g/kg均可显著对抗丙酸睾酮的作用,显著降低其前列腺指数(P<0.05或P<0.01)。大鼠在注射丙酸睾酮注射液5.0 mg/kg 8 w后,与正常对照组比较,模型对照组前列腺指数、腺上皮所占的比例均显著升高(P<0.05或P<0.01);与模型对照组比较,模型大鼠给予前列爽3.0 g/kg、6.0 g/kg可显著降低前列腺指数(P<0.05),前列爽6.0 g/kg还可显著降低其腺上皮所占的面积比例。大鼠在连续注射丙酸睾酮注射液5 mg/kg 5 w后,与正常对照组比较,模型对照组前列腺指数显著增加(P<0.01),膀胱内压对去氧肾上腺素的敏感性显著增加(P<0.01);与模型对照组比较,前列爽3.0 g/kg和6.0 g/kg可以显著对抗丙酸睾酮增加前列腺指数的作用,前列爽6.0 g/kg还可显著对抗去氧肾上腺素致前列腺增生大鼠膀胱内压升高(P<0.05或P<0.01)。结论:前列爽对前列腺增生有较好的防治作用,既能降低前列腺指数,改善前列腺增生所致的静力性膀胱出口梗阻;也能对抗去氧肾上腺素,降低大鼠膀胱内压,缓解前列腺增生所致的动力性膀胱出口梗阻。其作用机理可能与其对抗雄激素、肾上腺素能α受体有关,详细相关机制有待进一步深入研究。Objective:To evaluate the prevention and treatment effect of Qianlieshuang on prostatic hyperplasia and its mechanism.Methods:Prostatic hyperplasia mice model and rats model were established.After orally administrated in mice(8.0,4.0,2.0 g/kg doses)and rats(1.5,3,6 g/kg doses),prostates were collected and weighed,then the effect of Qianlieshuang on the prostate index,the area of glandular epithelium and pathological changes were studied.Rats with modeling for 5 weeks and administration for 5 weeks were taken,0.5 h after last administration,to measure their intravesical pressures before and after 2.0 mg/kg phenylephrine hydrochloride was intravenously injected,then prostates were collected to calculate the index.Results:After 3 weeks of testosterone propionate injection into mice,compared with the control group,the prostate index was increased in the model group(p<0.01).Compared with the model group,the prostate indexes were significantly decreased in Qianlieshuang groups(8.0,4.0,2.0 g/kg),which suggested an anti-prostatic hyperplasia effect(p<0.05 or p<0.01).After 8 weeks of injection with 5.0 mg/kg testosterone propionate in rats,compared with the control group,the prostate index and the area of glandular epithelium were significantly increased in the model group(p<0.05 or p<0.01).Compared with the model group,the prostate indexes were significantly decreased in 3.0 g/kg and 6.0 g/kg Qianlieshuang groups(p<0.05),and the area of glandular epithelium was significantly decreased in 6.0 g/kg group.After 5 weeks of 5.0 mg/kg testosterone propionate injection,compared with the control group,the prostate index was significantly increased in the model group(p<0.01),the sensitivity of intravesical pressure to phenylephrine was induced(p<0.01).Compared with the model group,the prostate indexes were significantly decreased in Qianlieshuang groups(3.0 g/kg,6.0 g/kg),also the sensitivity of intravesical pressure to phenylephrine was reduced in 6.0 g/kg Qianlieshuang group(p<0.05 or p<0.01).Conclusion:Qianlieshuang has the go
