摘　　要：蔡炳勤教授从事周围血管疾病中医临床五十余年,对筋瘤、臁疮提出"因虚致瘀、湿瘀阻络"的观点,主张内外结合,内服药多以益气活血、祛湿通络为法,外用药组方重视三点:中医理法辨证、透皮吸收的效果、现代药理研究结果。蔡教授的外用药少而精,量大效宏。筋瘤早期用蔡氏筋瘤洗方(大黄、乌梅等)熏洗,配合刺络放血以缓解症状,伴瘀积性皮炎用清热凉血解毒,或养血润燥止痒之品。臁疮外治法包含中药熏洗、局部清创、药物湿敷或膏药外敷、针刺等一系列治法,循中医局部辨治思路,分期使用,值得学习。Professor Cai Bingqin has been engaged in traditional Chinese medicine of peripheral vascular diseases for more than 50 years.The viewpoint of"blood stasis caused by deficiency and dampness stasis obstructing collaterals"was put forward for varicose veins and ecthyma.It was advocated that internal medicine should be combined with internal medicine,with the method of invigorating qi and activating blood circulation and clearing damp dredging collaterals.External medicine prescription paid attention to three points:syn-drome differentiation by TCM,effect of transdermal absorption,and results of modern pharmacological research.Professor CAI’s external medicine although in smaller quantity and precise but has greater effect.Jin tumor early treated by CAI jin tumor washing formula(rhubarb,dark plum,etc.)and thorn collateral bleeding helps to relieve symptoms.Chinese medicine formula also accompanied by silt dermatitis with qingreliang blood detoxification or nourishing blood herbs to improve the curative effect.External treatment method of ec-thyma included a series of treatment methods including fumigation and washing of traditional Chinese medicine,local detoxification,wet compress or plaster external application,acupuncture,etc,according to the traditional Chinese medicine differentiation and treatment ideas,used in stages,it is worth learning.
