摘 要:目的探讨基质金属蛋白酶-9 (matrix metalloproteinase-9,MMP-9)、基质金属蛋白酶抑制剂-1(matrix metalloproteinase inhibitor-1,TIMP-1) 及其比值在评估膝骨关节炎 (knee osteoarthritis,KOA) 患者病情严重程度中的价值。方法选择我院 2016 年 5 月至 2019 年 8 月期间确诊为 KOA 的 97 例患者作为研究对象,其中男 55 例,女 42 例;平均年龄 (62.7±6.8) 岁。97 例 KOA 患者根据 Kellgren-Lawrence (K-L) 分级为 I 级 (n=24 例)、II 级 (n=25 例)、III 级 (n=23 例)、IV 级 (n=25 例),同期选择 30 例无关节炎表现的膝关节运动型损伤手术患者作为对照组。比较 KOA 组及对照组,不同 K-L 分级患者血清及关节液 MMP-9、TIMP-1、MMP-9 / TIMP-1 的差异性,并分析 K-L 分级与血清及关节液 MMP-9、TIMP-1、MMP-9 / TIMP-1 的相关性。结果 KOA 组患者血清及关节液 MMP-9、TIMP-1、MMP-9 / TIMP-1 均明显高于对照组,差异有统计学意义 (P<0.05);不同 K-L 分级组 KOA 患者血清及关节液 MMP-9、TIMP-1、MMP-9 / TIMP-1 比较差异有统计学意义 (P<0.05),且随着病情程度加重,血清及关节液 MMP-9、TIMP-1、MMP-9 / TIMP-1 均明显升高,差异有统计学意义 (P<0.05);K-L 分级与血清 MMP-9、TIMP-1、MMP-9 / TIMP-1 (r=0.372,0.397,0.475)及关节液 MMP-9、TIMP-1、MMP-9 / TIMP-1 (r=0.406,0.442,0.581) 均呈正相关性关系 (P<0.05)。结论血清及关节液 MMP-9、TIMP-1、MMP-9 / TIMP-1 与 KOA 患者病情严重程度存在密切联系,其中尤以关节液 MMP-9 / TIMP-1 评估 KOA 患者病情严重程度的价值最优。Objective To investigate the value of matrix metalloproteinase-9 (MMP-9),matrix metalloproteinase inhibitor-1 (TIMP-1),and their ratio in the severity evaluation of knee osteoarthritis (KOA).Methods Ninety-seven patients with KOA were analyzed from May 2016 to August 2019,including 55 males and42 females with an average age of (62.7±6.8) years.All patients were classified as grade I (n=24 cases),grade II(n=25 cases),grade III (n=23 cases) and grade IV (n=25 cases) according to Kellgren-Lawrence (K-L).At the same time,30 patients with sports injury (no KOA) were selected as the control group.Differences of MMP-9,TIMP-1,MMP-9 / TIMP-1 in serum and joint fluid were compared between KOA group and control group,and the correlation between K-L and MMP-9,TIMP-1,MMP-9 / TIMP-1 in serum and joint fluid was analyzed.Results The levels of MMP-9,TIMP-1 and MMP-9 / TIMP-1 in serum and joint fluid of KOA group were higher than those of the control group,and the differences were statistically significant (P < 0.05).The levels of MMP-9,TIMP-1,and MMP-9 /TIMP-1 in serum and joint fluid were significantly different among different K-L grades (P < 0.05).Higher MMP-9,TIMP-1,and MMP-9 / TIMP-1 in serum and joint fluid indicated severer aggravation of the disease,and the differences were statistically significant (P < 0.05).There was a positive correlation between K-L and MMP-9,TIMP-1,MMP-9 /TIMP-1 (r=0.372,0.397,0.475) in the serum and MMP-9,TIMP-1,MMP-9 / TIMP-1 (r=0.406,0.442,0.581) in the joint fluid (P < 0.05).Conclusions MMP-9,TIMP-1,and MMP-9 / TIMP-1 in serum and joint fluid have close correlation to the severity of KOA,which can be used as an indicator to evaluate the severity of KOA.
