摘 要:目的探讨女性外阴和肛周人乳头瘤病毒(HPV)感染状况和相互间影响。方法采用横断面研究,对入组的356例女性同时采集外阴和肛周脱落细胞进行HPV分型检测,并分析外阴与肛周各亚型感染情况。结果356例患者中HPV阳性196例,感染率为55.06%,外阴和肛周部位总的HPV、高危型HPV及低危型HPV检出例数依次为187例、162例、56例和118例、101例、40例,阳性检出率依次为52.53%、45.51%、15.73%和33.15%、28.37%、11.24%,外阴与肛周相比,总HPV检出率和高危型HPV检出率明显增加,且差异有统计学意义(P<0.05)。外阴与肛周检测结果相同者共233例,检出一致率65.45%,其中两部位均为阴性者160例,两部位同时为阳性且感染亚型一致共73例。检测结果不一致的123例患者中,外阴HPV检出亚型数量高于肛周者100例,肛周检测率及检出亚型数高于外阴者18例,两部位检测结果不完全相同者5例。结论由于外阴和肛周HPV感染率均较高,且存在相互影响,提示在临床工作中应关注女性外阴和肛周HPV的感染。Objcetive To investigate the genotyping of HPV and interaction effect between female vulva and crissum. Methods Using cross-sectional method to analyze the differences in HPV prevalence between female vulva and crissum, HPV genotypes were determined in 356 women with exfoliated cells from the vulva and crissum. Results In the all 356 patients, the cases of HPV-positive were 196 and the infectious rate was 55.06%. The cases who were HPV-positive in the vulva and crissum were 187 and 118 respectively, and the positive detection rate were 52.53% and 33.15%. In addition, the prevalence rates of high-risk HPV were 45.51% and 28.37%, while the rates of low-risk HPV were 15.73% and 11.24%. There were significant differences between the two sites in HPV and high-risk HPV prevalence rate(P<0.05). There were 233 patients(65.45%) who had the same HPV test results in the vulva and crissum, among them, 160 patients were negative while 73 patients were positive in both sites. There were 123 patients who had the different HPV test results in the two sites, among them, 100 patients had more HPV genotyping in vulva while 18 patients had more HPV genotyping in crissum. Besides, there were 5 patient had not exactly same HPV test results. Conclusion Due to the high HPV infection rate and interaction effect in the vulva and crissum, the HPV infection of vulva and crissum should be paid more attention.
