二胎剖宫产待产妇服务需求现状及其影响因素

摘　　要：目的:调查二胎剖宫产待产妇服务需求现状,并分析其影响因素,为提供有针对性的围产期服务需求提供理论参考与实践指导。方法:通过便利抽样法,于术前1天运用孕妇服务需求调查问卷对225名二胎剖宫产待产妇进行问卷调查,采用logistic回归分析进行统计分析。结果:二胎剖宫产待产妇孕期对各项服务的需求率均在70%以上,排在前5位的服务需求依次是新生儿正常生理现象的介绍(96.73%)、新生儿正确喂养方法的讲解及演示(94.39%)、术后疼痛控制相关知识(94.39%)、产褥期观察与自我护理指导(93.46%)和孕晚期陪伴(91.12%)。结婚3年内(P=0.006)、夫妻之间有时吵架(P=0.036)、父母对生二胎持支持态度(P=0.013)是影响二胎剖宫产产妇服务需求的重要因素。结论:二胎剖宫产待产妇对围产期服务的需求程度普遍较高,结婚时间短、夫妻关系不良、父母对生育二胎支持程度高者对服务需求更高,建议重点关注。Objective:To investigate the service demands of parturient mothers who will give cesarean birth to second child and analyze the influencing factors,so as to provide theoretical reference and practical guidance for ensuring targeted perinatal service demands.Methods:The convenience sampling method was used to conduct a questionnaire survey on 225 parturient mothers one day before the surgery.The logistic regression analysis was used for statistical analysis.Results:More than 70%of the parturient mothers have needs on all of the service options,and the top 5 popular services are"the introduction of the normal physiological phenomena of newborns"(96.73%),"the correct feeding methods for newborns"(94.39%),"knowledge about postoperative pain control"(94.39%),"puerperium observation and self-care guidance"(93.46%),and"companionship during the third trimester"(91.12%).Three factors,"being married within 3 years"(P=0.006),"having quarrels between couples sometimes"(P=0.036),and"parents'supportive attitude towards the second child"(P=0.013),are significantly associated with the service demands of parturient mothers who will give cesarean birth to second child.Conclusion:The demand of perinatal services for parturient mothers who will give cesarean birth to second child is generally high.More attention may be needed to paid to those mothers with short marriage time,poor relationship between couples,and parents highly supporting for second childbearing because of their higher demands for perinatal services.
