常规MRI及DW-MRI检查对肺癌的鉴别诊断价值分析

摘　　要：目的探讨常规磁共振(MRI)及扩散加权成像(DW)-MRI检查对肺癌的鉴别诊断价值。方法选取2017年2月-2019年8月就诊于我院的肺部肿瘤患者90例,均接受常规MRI及DW-MRI检查。以病理诊断结果为金标准,分析常规MRI及DW-MRI诊断肺癌效能,并比较肺癌和肺部良性肿块患者肿块与胸髓信号强度比值(LSR)评分和表观扩散系数(ADC)值差异。结果90例患者经病理诊断确诊为肺癌54例、肺部良性肿块36例;DW-MRI诊断肺癌特异度97.22%(35/36)、敏感度94.44%(51/54)、准确度为95.56%(86/90),高于常规MRI的75.00%(27/36)、81.48%(44/54)、78.89%(71/90),差异有统计学意义(P<0.05);肺癌患者LSR评分高于肺部良性肿块患者,ADC值低于肺部良性肿块患者,差异有统计学意义(P<0.05)。结论与常规MRI相比,DW-MRI诊断肺癌的特异度、敏感度和准确度较高,利于鉴别诊断肺癌。Objective To explore the value of conventional magnetic resonance(MRI)and diffusion weighted imaging(DW)-MRI in the differential diagnosis of lung cancer.Methods From February 2017 to August 2019,90 patients with lung tumor were selected and received conventional MRI and DW-MRI.Based on the results of pathological diagnosis as the gold standard,the diagnostic efficacy of conventional MRI and DW-MRI for lung cancer was analyzed,and the differences of LSR and ADC between lung cancer and benign lung mass were compared.Results There were 54 cases of lung cancer and 36 cases of benign lung mass diagnosed by pathology in 90 patients.The specificity,sensitivity and accuracy of DW-MRI were 97.22%(35/36),94.44%(51/54),95.56%(86/90),higher than 75.00%(27/36),81.48%(44/54)and 78.89%(71/90)of conventional MRI,and the difference was statistically significant(P<0.05).The LSR score of lung cancer patients was higher than that of benign lung cancer patients.The ADC value of lung cancer patients was lower than that of patients with benign lung mass,the difference was statistically significant(P<0.05).Conclusion Compared with conventional MRI,DWMRI has higher specificity,sensitivity and accuracy in the diagnosis of lung cancer,which is helpful for the differential diagnosis of lung cancer.
