颈部高频超声在颈段食管疾病中的临床价值

摘　　要：目的探讨颈部高频超声在颈段食管疾病中的临床价值。方法回顾性分析景德镇市第二人民医院2018年6月至2019年12月收治的颈段食管疾病患者25例,按疾病类型不同分为3组:食管癌组10例,食管穿孔并周边炎性包块组7例,咽食管憩室组8例,分析各疾病组声像图特征,并比较各疾病组食管壁增厚情况。结果各疾病组声像图共同特征为食管周边不均质含气体肿物,与咽食管憩室组比较,食管穿孔并炎性包块组食管壁增厚明显,差异有统计学意义(P<0.05);与食管穿孔并炎性包块组比较,食管癌组食管壁增厚更为明显,差异有统计学意义(P<0.05)。结论虽然颈部高频超声不能全面评价颈段食管疾病,但是仍具有一定的临床价值,超声医师应充分认识颈段食管病变,避免误诊误治。Objective To explore the clinical value of high frequency ultrasound in cervical esophageal diseases.Methods Data of 25 patients with cervical esophageal diseases admitted to the second people’s Hospital of Jingdezhen from June 2018 to December 2019 were retrospectively analyzed.According to the types of diseases,these patients were divided into three groups:esophageal cancer group(n=10),esophageal perforation with peripheral inflammatory mass group(n=7),and pharyngeal esophageal diverticulum group(n=8).The ultrasonograhic features of each group were analyzed and the thicknes of esophageal wall was compored.Results The common characteristics of ultrasonography in the three groups were heterogeneous gas mass around the esophagus.Compared with esophageal perforation with peripheral inflammatory mass group,esophageal wall thickness decreased in pharyngeal esophageal diverticulum group but increased in esophageal cancer group(P<0.05).Conclusion Although high frequency ultrasound cannot comprehensively evaluate cervical esophageal diseases,it still has some clinical value.Sonographers should fully understand the cervical esophageal lesions to avoid misdiagnosis and mistreatment.
