4P医学模式社区护理对脑卒中高危人群预防效果分析

摘　　要：目的:探讨以4P医学模式为基础的社区护理对脑卒中高危人群一级预防效果的影响。方法:对某社区的常住居民进行基线调查,筛选出218例脑卒中高危人群作为研究对象,随机分为对照组与实验组。对照组进行常规健康宣教,实验组在对照组的基础上进行以4P医学模式为基础的社区护理。干预1年后,观察记录两组患者的危险因素管理情况以及自我管理能力。结果:干预之前两组患者的危险因素管理情况以及自我管理能力差异无统计学意义(P>0.05)。干预之后实验组患者的危险因素管理情况以及自我管理能力优于对照组,差异具有统计学意义(P<0.05)。结论:以4P医学模式为基础的社区护理对脑卒中高危人群一级预防效果显著,能够对患者的危险因素管理效果以及自我管理能力进行改善,在临床护理中值得进一步推广应用。Objective:To explore the impact of community nursing based on the 4P medical model on the primary prevention effect of stroke high-risk populations;Methods:A baseline survey of permanent residents in a community was conducted,and 218 stroke high-risk populations were selected as the research objects and randomly divided into Control group and experimental group.The control group received regular health education,and the experimental group performed community care based on the 4P medical model on the basis of the control group.After one year of intervention,observe and record the risk factor management and self-management ability of the two groups of patients.Results:Before the intervention,there was no statistically significant difference in risk factor management and self-management ability between the two groups of patients(P>0.05).After the intervention,the difference in risk factor management and self-management ability of the experimental group was better than that of the control group,the difference was statistically significant(P<0.05).Conclusion:Community nursing based on the 4P medical model has a significant effect on the primary prevention of stroke high-risk groups,and can improve the risk factor management effect and self-management ability of patients.It is worthy of further promotion and application in clinical nursing.
