利拉鲁肽注射液联合盐酸吡格列酮片治疗肥胖型T2DM的效果及对内脏脂肪素、瘦素与胰岛素抵抗的影响

摘　　要：目的分析利拉鲁肽注射液联合盐酸吡格列酮片治疗肥胖型2型糖尿病(T2DM)的临床效果。方法将136例肥胖型T2DM患者按奇偶数分组法分为对照组和观察组,每组68例。对照组采用常规降糖基础治疗联合利拉鲁肽注射液治疗,观察组在对照组基础上采用盐酸吡格列酮片治疗。比较两组的临床效果。结果观察组血糖达标时间短于对照组,低血糖发生次数少于对照组(P<0.05);治疗后,两组患者的FPG、2 h PG水平及HbA1c、BMI均降低,且观察组低于对照组(P<0.05);治疗后,两组患者的内脏脂肪素、瘦素水平均下降,且观察组低于对照组(P<0.05);治疗后,两组患者的HOMA-IR均降低,HOMA-β均增加,且观察组优于对照组(P<0.05)。结论利拉鲁肽注射液联合盐酸吡格列酮片治疗肥胖型T2DM效果显著,可降低内脏脂肪素水平,改善胰岛素抵抗。Objective To analyze the clinical effect of liraglutide injection combined with pioglitazone hydrochloride tablets in the treatment of obese type 2 diabetes mellitus(T2DM).Methods A total of 136 patients with obese T2DM were divided into control group and observation group by odd and even grouping method,with 68 cases in each group.The control group was treated with routine hypoglycemic basic therapy combined with liraglutide injection,and the observation group was treated with pioglitazone hydrochloride tablets on the basis of the control group.The clinical effects of the two groups were compared.Results The time of blood glucose reaching the standard in the observation group was shorter than that in the control group,and the frequency of hypoglycemia in the observation group was less than that in the control group(P<0.05);after treatment,FPG,2 h PG levels and HbA1c,BMI of the two groups decreased,and those in the observation group were lower than the control group(P<0.05);after treatment,the levels of visfatin and leptin in the two groups decreased,and those in the observation group were lower than the control group(P<0.05);after treatment,HOMA-IR decreased and HOMA-βincreased in the two groups,and those in the observation group were better than the control group(P<0.05).Conclusion Liraglutide injection combined with pioglitazone hydrochloride tablets in the treatment of obese T2DM has a significant effect,which can reduce the level of visfatin and improve insulin resistance.
