摘　　要：目的观察磁共振成像(MRI)在不同临床分期布氏杆菌性脊柱炎(BS)患者中的表现及应用价值。方法回顾性分析我院经手术病理或临床实验室检查确诊的32例BS患者的临床及MRI图像资料,观察不同临床分期患者的MRI表现。结果9例急性期病例表现为椎体、部分椎间盘、椎旁及背部软组织不同程度的T1WI低信号、T2WI高或混杂信号,无骨质破坏,无椎间隙狭窄。14例亚急性期病例椎体与部分椎间盘及周围软组织在T1WI及T2WI表现为不均匀信号,10例椎体骨质破坏并不同程度骨质增生、硬化,1例椎间隙轻度狭窄,3例椎管内脓肿。9例慢性期病例表现为椎体与椎间盘T1WI呈等、低或混杂信号,T2WI椎体呈混杂信号,9例均无骨质破坏,1例椎间隙轻度狭窄,椎旁及腰背部软组织未见明显异常。结论MRI在不同临床分期的BS患者中的表现具有一定的特征性,对临床治疗及疗效评价有重要的指导意义。Objective To observe the manifestations and application value of magnetic resonance imaging(MRI)in different clinical stages of Brucella spondylitis(BS)patients.Methods The clinical and MRI image data of 32 patients with BS diagnosed by surgical pathology or clinical laboratory examination in our hospital were retrospectively analyzed,and the MRI manifestations in different clinical stages patients were observed.Results Nine cases of acute stage showed different degrees of low T1WI signal,high or mixed T2WI signal in the vertebral body,part of the intervertebral disc,paravertebral and dorsal soft tissue,without bone destruction or vertebral space stenosis.In 14 cases of subacute stage,the vertebral body,part of intervertebral disc and surrounding soft tissue showed uneven T1WI and T2WI signal,10 cases of vertebral bone destruction and varying degrees of bone hyperplasia and sclerosis,1 case of mild narrowing of intervertebral space,3 cases of intraspinal abscess.Nine cases of chronic stage showed equal,low or mixed T1WI signal of vertebral body and intervertebral disc,and mixed T2WI signal of vertebral body,no bone destruction was found in the 9 patients.One case showed mild narrowing of intervertebral space,and no obvious abnormality was found in paravertebral and lumbar back soft tissues.Conclusion MRI has certain characteristics in different clinical stages of BS patients,which has important guiding significance for clinical treatment and curative effect evaluation.
