摘 要:目的探讨尿微量白蛋白与肌酐比值(ACR)、β-痕迹蛋白(BTP)、脂联素(APN)水平与妊娠期高血压(HDP)病情程度及与妊娠结局的关联性。方法选取2018年10月至2019年7月我院收治的HDP患者95例作为研究组,另选取同期正常妊娠女性95例为对照组。均检测血清BTP、APN水平及尿微量白蛋白、肌酐水平,计算ACR值。比较两组及不同病情程度HDP患者ACR、BTP、APN水平,分析其与HDP、病情程度关联性,随访统计两组妊娠结局,对比不同妊娠结局患者ACR、BTP、APN水平。结果研究组ACR、BTP高于对照组,APN低于对照组(P<0.05);ACR、BTP、APN与HDP具有显著关联性(P<0.05);不同病情程度HDP患者ACR、BTP、APN间存在显著差异(P<0.05),且随病情加重ACR、BTP呈升高趋势,APN呈降低趋势(P<0.05);ACR、BTP与HDP病情程度呈正相关,APN与HDP病情程度呈负相关(P<0.05);研究组不良妊娠结局发生率17.89%(17/95)高于对照组7.37%(7/95)(P<0.05);不良妊娠结局者ACR、BTP高于正常妊娠者,APN低于正常妊娠者(P<0.05)。结论ACR、BTP、APN水平与HDP病情程度具有相关性,根据其水平变化可间接评估HDP病情程度,并为临床诊治、妊娠结局评估提供参考。Objective To investigate the correlation of levels of urine micro-albumin and creatinine ratio (ACR),serum β-tracein (BTP) and adiponectin (APN) with the severity of hypertension during pregnancy (HDP) and pregnancy outcomes.Methods From October 2018 to July 2019,95 patients with HDP in our hospital were selected as a study group,and 95 normal pregnant women during the same period were selected as a control group.The serum BTP and APN levels were detected and urine micro-albumin and creatinine levels were measured to calculate ACR values.The levels of ACR,BTP and APN of two groups and HDP patients with different disease levels were compared,and their correlation with disease levels was analyzed.The pregnancy outcomes of the two groups were followed up and the levels of ACR,BTP and APN were compared among patients with different pregnancy outcomes.Results The levels of ACR and BTP were higher and the APN level was lower in the study group than those in the control group ( P <0.05).ACR,BTP,APN and HDP were significantly related ( P <0.05).There were significant differences in ACR,BTP and APN in HDP patients with different levels of disease ( P <0.05).ACR and BTP showed an increasing trend while APN showed a decreasing trend with the worsening of the disease ( P <0.05).ACR,BTP and HDP were positively correlated with the disease degree of HDP while APN and HDP were negatively correlated with the disease degree of HDP ( P <0.05).The incidence of adverse pregnancy outcomes in the study group was 17.89% (17/95) that was higher than 7.37% (7/95) in the control group ( P <0.05).The levels of ACR and BTP were higher and the APN level was lower in patients with adverse pregnancy outcomes than those of normal pregnancy ( P <0.05).Conclusion The levels of ACR,BTP and APN are highly correlated with the severity of HDP.According to its level change,the disease degree of HDP can be assessed indirectly.It can provide a reference for clinical diagnosis and treatment and assessment of pregnancy outcome.
