摘 要:目的探讨基于镜像神经元系统的言语动作观察训练对脑卒中后Broca失语患者的疗效。方法将符合条件的40例患者,采用随机数字表法将其分为试验组和对照组各20例。对照组采用常规言语治疗,每次30分钟,每日1次;试验组在对照组的基础上,增加观看言语动作视频,每天1次,每次训练20分钟。两组均为每周5天,疗程为4周。于治疗前、治疗4周后,采用西方失语成套测验(WAB)对患者进行评定。结果治疗4周后,两组WAB评分及失语商(AQ)值均较治疗前有明显提高(P<0.01);试验组WAB评分及AQ值前后差值的改变显著高于对照组,差异有统计学意义(P<0.01)。结论基于镜像神经元的言语动作观察与言语训练的协同效果明显优于常规的语言康复治疗,对于脑卒中后Broca失语的言语障碍恢复具有积极作用。Objective To investigates the effect of mirror neuron-based speech action observation training in the treatment of post-stroke Broca aphasia.Methods Forty patients who met the inclusion criteria were divided into an experimental group and a control group using random number table,20 in each group.The control group underwent conventional speech therapy for 30 minutes a session,1 session a day,5 days a week for a total course of 4 weeks.The experimental group received speech action observation training on top of conventional speech therapy for 20 minutes a session,1 session a day,5 days a week for a total course of 4 weeks.The linguistic skills of both groups were assessed before and after 4 weeks of treatment using Western aphasia battery(WAB).Results After 4 weeks of treatment,both groups showed improved WAB scores and aphasia quotient(AQ)(P<0.01).Compared to the control group,the experimental group showed a more significant increase in WAB scores and AQ after treatment(P<0.01).Conclusion The clinical outcome of the treatment scheme that combines the mirror neuron-based speech action observation training and conventional speech therapy is better than the conventional speech therapy alone in the language rehabilitation.This new treatment scheme has a positive impact on the recovery of speech functions in patients with post-stroke Broca aphasia.
