摘 要:外科吻合器的设计及应用中对生物固体的挤压或压榨时,生物固体会发生什么变化?在外科吻合器应用中有何影响?目前很少有人对生物固体(人体的内脏组织、脂肪结缔组织等)的流变作具体的应用研究。文章针对生物固体流变学导出一个显著的黏弹性特征,针对黏弹性在生物固体上所具体表现出的三大变形特点(双阶段性、蠕变性、顺应性),在理解3个变形特点后,了解了吻合器在设计及应用中所产生的压力性压榨损伤和压力性爆破损伤的原因。笔者在外科吻合器的专有领域中建立了一个初步的吻合器设计理论,可以将器械调节间隙的方式来应对不同的生物固体厚度,也可应用吻合器的不同固定间隙方式来应对不同的生物固体厚度(不同缝钉成型高度)。在临床上能了解生物固体在流变时的3个变形特点,根据3个特点导出一系列吻合器正确的操作方法(如器械的选择、等待的时间、缓慢的击发器械、吻合器上的最小间隙等),消除或避免手术并发症的发生。What are the changes in the design and application of surgical stapler when squeezing or squeezing biosolid?What are the effects brought by the changes in surgical stapler applications?At present,few people study the rheology of bio-solid(visceral tissue,adipose connective tissue,etc.).According to bio-solid rheology,this paper derives a remarkable viscoelasticity characteristic.According to the viscoelasticity in bio-solid,there are three deformation characteristics(double stage,creep deformation and compliance).After understanding the three deformation characteristics,we know the reasons of pressure squeezing damage and pressure blasting damage in the design and application of stapler.The author tries to establish a preliminary theory of stapler design in the proprietary field of surgical stapler.For example,the device can be used to adjust the gap to cope with different bio-solid thicknesses.Stapler can also be used to cope with different bio-solid thicknesses(height of different staples).Three deformation characteristics of bio-solid during rheology can be understood in clinic.According to three characteristics,a series of correct operation methods of stapler(such as device selection,waiting time,slow firing device,minimum gap on stapler)can be derived to eliminate or avoid complications.
