摘 要:目的评估人工耳蜗植入(CI)对语前聋患儿听觉言语康复和声调的影响。方法选择2014年3月~2019年3月在延安大学附属医院和解放军总医院接受CI的80例语前聋患儿。所有患儿随访时间为1年。在开机前、康复3个月、6个月、9个月、12个月,利用听觉行为分级(CAP)和言语可懂度分级(SIR)量表评估听觉言语康复效果。参照言语噪声下汉语普通话测试系统制定本研究的音频词汇表,测试患儿的声调水平。结果患儿在术后CAP和SIR评分逐渐提高,CAP评分在术后12个月提升最快,SIR评分在术后9个月提升最快,对比均有统计学意义(P<0.05);两个年龄段患儿在二声和三声时出错率高,其中2~4岁组在第一次测评时三声正确率为0,与第一次测评相比,患儿在第二次测评时各声调的正确率明显提高,差异具有统计学意义(P<0.05);4岁以上组患儿在第二次测评时各声调的正确率有提高,与第一次测评对比无统计学意义(P>0.05)。2~4岁组患儿各声调正确率的提升水平显著优于4岁以上组(P<0.05)。结论语前聋患儿在人工耳蜗植入术后,听觉言语能力和声调发音能力逐步改善,在术后9~12个月进步明显。Objective To evaluate the effect of cochlear implantation(CI)on hearing rehabilitation and intonation in children with prelingual deafness.Methods Eighty pre-verbal deaf children who received CI in Affiliated Hospital of Yan'an University and PLA General Hospital(301)from March 2014 to March 2019 were selected.All children were followed up for 1 year.Before the start-up,3 months,6 months,9 months,and 12 months of rehabilitation,the auditory behavior rehabilitation(CAP)and speech intelligibility rating(SIR)scales were used to evaluate the effects of auditory language rehabilitation.The audio vocabulary list of this study was developed using the music perception test,includes a variety of sounds and intonation to test the child's verbal clarity.Results The CAP and SIR scores of the children gradually increased after the operation.The CAP score increased fastest at 12 months after the operation,and the SIR score increased fastest at the 9 months after the operation,and the comparison was statistically significant(P<0.05).When evaluating the accuracy of tonal pronunciation,children of two age groups had a higher error rate in the second and third tones;among them,the accuracy of the third tones in the 2~4 year old group was 0 in the first evaluation,which was the same as the first evaluation.In comparison,the correct rate of each tone of children in the second assessment was significantly improved,and the difference was statistically significant(P<0.05).The correct rate of each tone of children in the group of children over 4 years of age increased in the second assessment high,but the contrast is not statistically significant(P>0.05).The improvement rate of each tone of children in the 2~4 year old group was significantly better than that in the group over 4 years old,and the difference was statistically significant(P<0.05).Conclusion Children with pre-verbal deafness gradually improve their auditory language ability and tone pronunciation ability after cochlear implantation,and they have made significant progress 9
