摘 要:目的通过孕早期血清甲状腺素水平测定,研究妊娠期糖尿病与甲状腺功能的相关性。方法选取2018年6月至2019年6月在绵阳市中心医院产检的单胎孕妇1851例,在孕早期进行抗甲状腺过氧化酶抗体(A-TPO)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)检测;于孕中期进行口服葡萄糖耐量试验(OGTT),结果异常者为妊娠期糖尿病(GDM)组,共184例,正常者为非妊娠期糖尿病GDM组,共1667例,分析两组孕妇甲状腺激素水平的差异以及甲状腺激素水平与OGTT各时相血糖的相关性。结果非GDM孕妇与GDM孕妇早孕期A-TPO(28.0 vs.30.0)及TSH(1.49 vs.1.46)比较,差异无统计学意义(P>0.05),两组患者FT3(0.33 vs.0.36)及FT4(1.06 vs.1.04)水平比较,差异有统计学意义(P<0.05),且FT3、FT4水平与OGTT各时相血糖具有相关性(P<0.05)。结论孕早期甲状腺激素水平中FT3、FT4与GDM存在相关性,可一定程度预测GDM的发生,但FT3、FT4可能并不是GDM的独立预测因素。Objective To study the relationship between gestational diabetes mellitus and thyroid function by measuring serum thyroxine level in early pregnancy.Methods A total of 1851 singleton pregnant women were selected from June 2018 to June 2019 in Mianyang Central Hospital.Anti thyroid peroxidase antibody(A-TPO),free triiodothyronine(FT3),free thyroxine(FT4)and thyroid stimulating hormone(TSH)were detected in the first trimester of pregnancy;oral glucose tolerance test(OGTT)was conducted in the second trimester.The results showed that the normal subjects were non GDM group(1667 cases)The abnormal cases were GDM group(184 cases).The difference of thyroid hormone level between the two groups and the correlation between thyroid hormone level and OGTT blood glucose were analyzed.Results There was no statistically significant difference in A-TPO(28.0 vs.30.0)and TSH(1.49 vs.1.46)between non GDM pregnant women and GDM pregnant women in early pregnancy period.There was significant difference in the levels of FT3(0.33 vs.0.36)and FT4(1.06 vs.1.04)between non GDM pregnant women and GDM pregnant women(P<0.05),and the levels of FT3 and FT4 were correlated with blood glucose in each phase of OGTT(P<0.05).Conclusion FT3 and FT4 are correlated with GDM in early pregnancy,which can be used to predict the occurrence of GDM,but FT3 and FT4 may not be independent predictors of GDM.
