超声造影对脂肪肝背景下闭合性肝损伤的诊断价值

摘　　要：目的探讨超声造影对脂肪肝背景下闭合性肝损伤的诊断价值。方法回顾研究54例脂肪肝患者临床可疑闭合性肝损伤的常规超声与超声造影资料,并以增强CT为标准进行分析。结果54例脂肪肝患者经增强CT证实闭合性肝损伤34例,常规超声诊断的灵敏度为73.5%(25/34)、特异度为45.0%(9/20)、准确率为62.9%(34/54),超声造影诊断的灵敏度为97.0%(33/34)、特异度为100%(20/20)、准确率为98.1%(53/54),两者之间差异有统计学意义(P<0.05)。并且在急性期(受伤24 h内)、轻至中度损伤、右后叶病灶方面,超声造影的诊断准确率均显著高于常规超声(P<0.05)。结论超声造影能够显著的提高脂肪肝背景下闭合性肝损伤的诊断效能,尤其在受伤24 h内更能准确检出肝损伤灶。Objective To asscess the application of contrast-enhanced ultrasound(CEUS)in diagnosis of liver traumatic patients with fatty liver disease.Methods Based on contrast-enhanced CT,US and CEUS were used and analyzed retrospectiverly in 54 cases of suspected liver traumatic patients with fatty liver disease.Results To compared with those of Contrast-enhanced CT,34 cases of liver trauma were confirmed.US displayed sensitivity 73.5%(25/34),specificity 45.0%(9/20),and accuracy 62.9%(34/54)in the diagnosis of liver trauma with fatty liver.And CEUS had sensitivity 97.0%(33/34),specificity 100%(20/20),and accuracy 98.1%(53/54).The occurrence ratio was significantly different between these two entities(P<0.05).In the groups of acute period(injured within 24 hours),minor-moderate lesions,right posterior liver,the accuracy ratio was significantly different between US and CEUS(P<0.05).Conclusions Compared with US,CEUS were demonstrated with higher sensitivity,specificity and accuracy,and would have more advantages in the diagnosis of liver trauma with fatty liver,especially in the period injured within 24 hours for liver trauma.
