盆腔器官脱垂患者不同体位超声测值差异及其对评估脱垂程度的影响

摘　　要：目的探讨盆腔器官脱垂(POP)患者最大Valsalva动作时站立位与仰卧位盆腔器官位置的差异,并评价不同体位超声测值对评估脱垂程度的影响。方法选择经盆腔器官脱垂定量评估(POP-Q)系统临床分度≥Ⅰ度的患者87例,分别在仰卧位和站立位行经会阴二维超声检查,记录检查耗时。测量患者最大Valsalva动作时盆腔器官的位置,以POP-Q系统为"金标准",绘制通过超声测值评估POP程度的受试者工作特征(ROC)曲线,计算曲线下面积,比较两种体位超声测值评价脱垂程度的诊断效能。结果最大Valsalva动作时,站立位检查耗时更长,站立位膀胱、子宫和直肠壶腹部的位置更低,差异具有统计学意义(P<0.05)。通过超声测值评估脱垂程度的ROC曲线显示站立位曲线下面积略高于仰卧位,但差异无统计学意义(P>0.05)。结论POP患者站立位时盆腔器官位置更低,但两种体位超声测值评估脱垂程度无明显差异。Objective To investigate the difference of pelvic organ position between standing position and supine position in patients with pelvic organ prolapse(POP).Methods 86 patients with POP-Q≥Ⅰgrade were selected.2 D ultrasonography was performed in supine position and standing position respectively.The position of pelvic organs was measured during the maximum valsalva maneuver.Taking the POP-Q system as the"gold standard",the ROC curve was drawn to evaluate the degree of POP by ultrasonic measurement.The AUC was calculated to compare the diagnostic efficacy of the ultrasonic values in two body positions.Results The time of standing position examination was longer,and the position of bladder,uterus,and rectum ampulla were lower in standing position(P<0.05).In the ROC curve evaluation of pop symptoms and the prediction of prolapse degree by ultrasonic values,the AUC of the standing position was slightly higher than that of the supine position,but the difference was not statistically significant(P>0.05).Conclusions The position of pelvic organs is lower when POP patients are standing,but there is no significant difference between the two positions in reflect the degree of POP.
