超声在局限型腱鞘巨细胞瘤诊断中的应用价值

摘　　要：目的探讨超声在局限型腱鞘巨细胞瘤诊断中的应用价值。方法回顾性分析了经手术和病理学诊断的25例局限性腱鞘巨细胞瘤的患者,均为单发病灶,术前均进行了超声和MRI检查,记录患者的姓名、性别、年龄和肿物的位置、大小、形态、内部回声、边界、有无钙化、有无液化和周围组织的关系、血流分级等情况。结果25例病理确诊的腱鞘巨细胞瘤常表现为肌腱旁、关节周围或包绕肌腱的实性肿物,回声多不均匀,形态规则或不规则,边界较清晰,可伴有钙化和液化。超声确诊20例,MRI确诊19例。超声与MRI在肿物诊断中差异无统计学意义。结论超声能够显示局限型腱鞘巨细胞瘤的特征性表现,早期诊断可减少对周围软组织的侵蚀和骨质破坏,超声较MRI操作简便、耗时少、经济实惠、实时动态、安全无辐射,可作为临床首选的术前检查方法。Objective To explore the value of ultrasound in the diagnosis of giant cell tumor of tendon sheath.Methods Twenty five patients with giant cell tumor of tendon sheath diagnosed by operation and pathology were analyzed retrospectively,all of them were single focus.Before operation,they were examined by ultrasonography and MRI.The patients′name,gender,age,location,size,shape,internal echo,boundary,calcification,liquefaction,relationship with surrounding tissue,blood flow classification were recorded.Results 25 cases of pathologically confirmed giant cell tumor of tendon sheath often showed solid masses beside tendon,around joint or around tendon,with irregular echo,regular or irregular shape,clear boundary,calcification and liquefaction.US confirmed 20 cases,MRI confirmed 19 cases.There was no significant difference between ultrasound and MRI in the diagnosis of tumor.Conclusions Ultrasound can show the characteristic features of localized giant cell tumor of tendon sheath.Early diagnosis can reduce the erosion of surrounding soft tissue and bone destruction.Compared with MRI,ultrasound is simple,time-consuming,economical,real-time dynamic,safe and radiation-free,and can be used as the first choice of preoperative examination.
