摘 要:目的探讨超声引导胎兔腹腔内注射乌司他丁(UTI)对N-硝基-左旋精氨酸甲酯(L-NAME)所致宫内生长受限(IUGR)胎兔肺发育的影响。方法采用L-NAME诱导建立IUGR模型。新西兰孕兔随机分为正常组、IUGR组和UTI组,孕25 d超声引导下UTI组胎兔腹腔注射UTI,正常组和IUGR组注射等量生理盐水。孕30 d行剖宫产得仔兔并测量体质量,取支气管肺泡灌洗液检测二棕榈酸磷脂酰胆碱(DPPC)含量和白细胞介素6(IL-6)含量,取肺组织行形态学观察及NO含量、SP-A表达水平的检测。结果与正常组相比,IUGR组仔兔体质量显著减少,辐射状肺泡计数(RAC)、DPPC含量、NO含量及SP-A表达水平降低,肺间质比例及IL-6含量增加(P<0.05)。与IUGR组相比,UTI组仔兔体质量无统计学差异,RAC、DPPC、NO含量及SP-A表达水平增加,肺间质比例及IL-6含量降低,但未达到正常水平(P<0.05)。结论超声引导胎兔腹腔内注射UTI可促进IUGR胎兔肺发育,提高肺表面活性物质生成,减少肺损伤,对兔肺发育有一定的促进和保护作用。Objective To investigate the effect of ultrasound-guided intraperitoneal injection of ulinastatin(UTI)on lung development in fetal rabbits with IUGR induced by N-nitrol-L-arginine(L-NAME).Methods The IUGR model was established by L-NAME.New Zealand pregnant rabbits were randomly divided into normal group,IUGR group and UTI group.UTI group rabbits were intraperitoneal injected with UTI under ultrasound-guided at 25 days of pregnancy,and normal group and IUGR group were injected with normal saline.Newborn rabbits were delivered by cesarean section at 30 days gestation and their weight was measured.The content of dipalmitoyl phosphatidylcholine(DPPC)and interleukin-6(IL-6)in bronchoalveolar lavage fluid were measured,and the lung tissues were examined for morphological observation,NO content and SP-A expression level.Results Compared with the normal group,the weight of newborn rabbits in the IUGR group was significantly reduced,the radiative alveolar count(RAC),DPPC content,NO content and SP-A expression level were decreased,the lung intermal ratio and IL-6 content were increased(P<0.05).Compared with the IUGR group,there was no significant difference in weight of newborn rabbits in the UTI group.The contents of RAC,DPPC,NO and SP-A were increased,the lung intermal ratio and IL-6 were decreased,but not to the normal level(P<0.05).Conclusions Intraperitoneal injection of UTI guided by ultrasound can promote lung development in IUGR fetal rabbits,improve the generation of surfactant in the lungs,reduce lung injury,promote and protect lung development in fetal rabbits.
