摘 要:目的探讨血清泛素羧基末端水解酶L1(UCH-L1)、神经元特异性烯醇化酶(NSE)指标水平与创伤性颅脑损伤患者预后的相关性。方法将2017年4月-2019年4月本院收治的129例创伤性颅脑外伤患者作为研究组,另选取同期本院健康体检者50例为健康对照组。通过酶联免疫吸附试验测定2组人员的血清UCH-L1、NSE指标水平,并经GOS评分(格拉斯哥预后评分)判定患者预后;选择Spearman相关性分析法分析血清UCH-L1、NSE指标水平和预后相关性。结果研究组患者的血清UCH-L1、NSE指标水平均高于健康对照组,差异有统计学意义(P<0.05);重型组、中型组创伤性颅脑损伤患者的血清UCH-L1、NSE指标水平均高于轻型组,且重型组高于中型组,差异有统计学意义(P<0.05);预后不良组患者的血清UCH-L1、NSE指标水平均高于预后良好组,差异有统计学意义(P<0.05);经Spearman相关性分析发现,血清UCH-L1、NSE指标水平和预后GOS评分显示为负相关(r值分别为-0.713、-0.751,P均<0.05)。结论监测血清UCH-L1、NSE指标水平能对创伤性颅脑损伤患者的病情严重程度进行反映,为临床医师评估创伤性颅脑损伤患者预后康复质量提供参考,即血清UCH-L1、NSE指标水平越高,创伤性颅脑损伤患者预后越差。Objective To investigate the correlation between serum ubiquitin carboxy terminal hydrolase L1( UCH-L1) and neuron specific enolase( NSE) levels and prognosis in patients with traumatic brain injury.Methods A total of 129 patients with traumatic craniocerebral trauma admitted to our hospital from April 2017 to April 2019 were enrolled as the study group.In addition,50 normal people who were in the hospital for physical examination at the same time were selected as the control group.The serum UCH-L1 and NSE levels were measured by enzyme-linked immunosorbent assay( ELISA),and the prognosis was determined by GOS score( Glasgow prognosis score),and the spearman correlation analysis was used to analyze serum UCH-L1,NSE index and prognosis correlation.Results The serum levels of UCH-L1 and NSE in the study group were higher than those in the control group( P<0.05).The serum UCH-L1 and NSE levels were higher in the severe and medium-sized patients with traumatic brain injury than that in the light group,and the severe group was higher than the medium group( P<0.05).The serum UCH-L1 and NSE levels in patients with poor prognosis were higher than those in the prognosis group( P<0.05).According to Spearman correlation analysis,serum UCH-L1,NSE index and prognosis GOS score showed a negative correlation( r=-0.713,r=-0.751,P<0.05).Conclusion The monitoring of serum UCH-L1 and NSE levels can reflect the severity of patients with traumatic brain injury,and provide a reference for clinicians to evaluate the prognosis quality of patients with traumatic brain injury,namely serum UCH-L1 and NSE levels.That is,the higher the serum UCH-L1 and NSE levels,the worse the prognosis of patients with traumatic brain injury.
