2018年杭州市手足口病与疱疹性咽颊炎流行特征分析

摘　　要：目的分析2018年肠道病毒流行高发期杭州市手足口病和疱疹性咽颊炎的流行情况及病原体构成。方法采集2018年4月-6月杭州市儿童医院收治的疱疹性咽颊炎和手足口病患儿的咽拭子或粪便标本,采用实时荧光定量RT-PCR法检测肠道病毒核酸。对2组疾病患儿的性别、年龄、感染病毒构成比进行分析。结果 77例疱疹性咽颊炎病例中,男、女比例为2.08∶1,5岁及以下病例占92.2%,肠道病毒Cox A10、Cox A6、Cox A16和其他未知类型的检出率分别为42.9%、15.6%、6.5%和35.1%,未检出EV71;285例手足口病病例中,男、女比例为2.47∶1,5岁及以下病例占97.5%,肠道病毒Cox A6、Cox A16、Cox A10、EV71和其他未知类型的检出率分别为55.4%、15.8%、14.4%、4.6%和9.8%。疱疹性咽颊炎的病例全为轻症,手足口病中有5.3%发展为重症。结论疱疹性咽颊炎检出以Cox A10为主,手足口病检出以Cox A6为主,两者的病原体构成不同,临床症状也有所区别。Cox A6较其他病原体发病年龄小。Objective To analyze the epidemic situation and pathogen composition of hand-foot-mouth disease( HFMD) and herpangina among children during the high incidence period of enterovirus in Hangzhou in 2018.Methods Throat swabs or stool samples of children with herpangina and HFMD were collected from April to June 2018 in Hangzhou Children’s Hospital.Enterovirus nucleic acid was detected by RT-PCR.The differences in genders,ages and the proportion of enterovirus subtypes between the two diseases were analyzed.Results The gender ratio was 2.08∶1 in 77 cases of herpangina.92.2% of children were under 5 years old.The constituent ratio of enterovirus subtypes Cox A10,Cox A6,Cox A16 and other types were 42.9%,15.6%,6.5% and 35.1%,respectively.EV71 was not appeared in herpangina.Among 285 cases of HFMD,the gender ratio was 2.47∶1 and children younger than 5 accounted for 97.5%.The constituent ratio of enterovirus subtypes Cox A6,Cox A16,Cox A10,EV71 and other types were 55.4%,15.8%,14.4%,4.6% and 9.8%,respectively.All cases of herpangina were mild,while 5.3% of HFMD conversion to severe.Conclusion Cox A10 was the main enterovirus subtype of herpangina,Cox A6 was the main enterovirus subtype of HFMD.The clinical symptoms were also different for the different constituent ratios of enterovirus subtypes.It should be noted that Cox A6 is more distributed in the younger age group.
