超声引导下隐神经阻滞在大隐静脉高位结扎剥脱术中的应用研究

摘　　要：目的探究超声引导下隐神经阻滞在大隐静脉高位结扎剥脱术中的应用效果。方法选择萍乡市人民医院2019年3月~2020年5月收治的行大隐静脉高位结扎剥脱术患者60例,采用随机数字表法分为两组,各30例。对照组行喉罩全身麻醉,观察组采用超声引导下隐神经阻滞。比较术后各时点疼痛评分[采用视觉模拟评分法(VAS)]以及不良反应发生情况。结果观察组术后1 h VAS评分为(1.98±0.37)分,术后8 h VAS评分为(3.43±0.35)分,对照组术后1 h VAS评分为(1.96±0.38)分,术后8 h VAS评分为(3.59±0.41)分,两组比较差异无统计学意义(P>0.05);观察组术后4 h VAS评分为(2.28±0.63)分,术后8 h VAS评分为(2.51±0.60)分,术后24 h VAS评分为(2.77±0.59)分,均低于对照组的(2.89±0.49)分、(3.03±0.45)分、(3.64±0.52)分,差异有统计学意义(P<0.05);观察组不良反应总发生率为6.67%,低于对照组的26.67%,差异有统计学意义(P<0.05)。结论超声引导下隐神经阻滞在大隐静脉高位结扎剥脱术中效果确切,阻滞效果显著,可有效缓解患者术后疼痛感,不良反应少、安全性高。Objective To explore the application effect of ultrasound-guided saphenous nerve block in high ligation and stripping of great saphenous vein.Methods From March 2019 to May 2020,a total of 60 patients with high ligation and stripping of great saphenous vein in Pingxiang People′s Hospital were selected and divided into two groups by the random number table method,30 cases in each group.The control group received laryngeal mask anesthesia,and the observation group received ultrasound-guided saphenous nerve block.The pain visual analogue scale(VAS)score at each time point after operation and adverse reactions were compared.Results The VAS score of the observation group was(1.98±0.37)points at the 1 h after operation,and at the 48 h after operation it was(3.43±0.35)points,and the VAS score at 1 h and 48 h after operation in the control group were(1.96±0.38)points and(3.59±0.41)points respectively,there was no significant difference between the two groups(P>0.05);the VAS scores of the observation group at 4,8,24 h after operation were(2.28±0.63)points,(2.51±0.60)points and(2.77±0.59)points,respectively,they were lower than those in the control group which were(2.89±0.49)points,(3.03±0.45)points and(3.64±0.52)points,the differences were statistically significant(P<0.05);the total incidence of adverse reactions of the observation group was 6.67%,it was lower than that in the control group which was 26.67%,the difference was statistically significant(P<0.05).Conclusion Ultrasound-guided saphenous nerve block in the application of high ligation and stripping of great saphenous vein has exact effect,remarkable blocking effect,can effectively relieve postoperative pain,with few adverse reactions and high safety.
