利培酮与奥氮平治疗精神分裂症临床疗效及对QTC间期延长的影响

摘　　要：目的:探讨利培酮与奥氮平治疗精神分裂症临床疗效及对OTC间期延长的影响。方法:选择2019年1月-2020年1月本院精神分裂症患者82例,运用随机表法分为两组各41例。A组给予患者利培酮片4~6 mg/次,1次/d;B组给予患者奥氮平口崩片15~20 mg/次,1次/d。两组疗程均为8周,比较两组患者治疗8周临床疗效,治疗前、治疗4周和治疗8周PANSS评分和QTC间期变化,及治疗前和治疗8周血钾和血镁变化。结果:A组与B组治疗8周总有效率分别为87.80%和85.37%,两组比较无明显差异(P>0.05);两组患者PANSS评分治疗4周和治疗8周较治疗前降低(P<0.05);两组患者PANSS评分治疗4周和治疗8周比较无明显差异(P>0.05);两组患者QTC间期治疗4周和治疗8周较治疗前延长(P<0.05);A组患者QTC间期治疗4周和治疗8周长于B组(P<0.05)。两组患者血钾和血镁治疗8周较治疗前无明显变化(P>0.05),两组患者血钾和血镁治疗8周比较无明显差异(P>0.05)。结论:利培酮与奥氮平治疗精神分裂症患者均获得良好临床疗效,且均会引起不同程度QTC间期延长,但利培酮QTC间期延长更为明显,因此需注意监测患者心电图变化。Objective:To investigate the efficacy of risperidone and olanzapine in the treatment of schizophrenia and the influence on the prolongation of QTc interval.Methods:82 patients with schizophrenia in our hospital from January 2019 to January 2020 were randomly divided into group A(41 cases)and group B(41 cases).Group A was given risperidone tablets(4-6 mg once a day),while group B was given olanzapine orally disintegrating tablets(15~20 mg once a day).The course of treatment was 8 weeks in both groups.The clinical efficacy of 8 weeks of treatment,PANSS score and QTc interval before treatment,4 weeks and 8 weeks of treatment,as well as the changes of serum potassium and magnesium were compared between the two groups.Results:The total effective rates of group A and group B were 87.80%and 85.37%,respectively,and there was no significant difference between the two groups(P>0.05).The PANSS score of the two groups decreased after 4 weeks and 8 weeks of treatment(P<0.05);there was no significant difference in PANSS score between the two groups after 4 weeks and 8 weeks of treatment(P>0.05).The QTc interval of group A was longer than that of group B(P<0.05).There was no significant difference in serum potassium and magnesium between the two groups after 8 weeks of treatment(P>0.05);there was no significant difference between the two groups after 8 weeks of treatment(P>0.05).Conclusion:Both risperidone and olanzapine have good curative effect in the treatment of schizophrenic patients.Both of them can cause different degree QTc interval prolongation,but risperidone QTc interval prolongation is more obvious,so it is necessary to pay attention to monitoring the ECG changes of patients.
