摘 要:目的了解深圳市坪山区手足口病的病原谱和柯萨奇病毒A组6型(CVA6)基因特征。方法收集坪山区2019年疑似手足口病的病例粪便或肛拭子样本,采用Real-time RT-PCR法进行肠道病毒核酸检测分型,选取其中的优势型别CVA6阳性样本进行VP1片段全长序列扩增、序列测定和系统进化分析。结果2019年共检测手足口病病例样本184份,检出肠道病毒通用型阳性165份,总阳性率为89.67%;其次为CVA6阳性120份(72.73%)、CVA16阳性40份(24.24%)、CVA10阳性8份(4.85%),其他肠道病毒阳性3份(1.82%),未检出肠道病毒71型(EV71)。同源性及系统进化分析显示,2019年深圳市坪山区24株CVA6 VP1基因核苷酸和氨基酸同源性分别为93.4%~99.9%和98.4%~100.0%,基因型为D3亚型,24株病毒VP1序列均分布于D3a.2分支上。结论2019年坪山区手足口病病原体以CVA6为主,D3a.2为其优势流行株。Objective To understand the pathogenic spectrum of hand,foot and mouth disease(HFMD)and the gene characteristics of coxsackievirus A6(CVA6)in Pingshan District of Shenzhen in 2019.Methods The feces or anal swab samples of suspected HFMD cases in Pingshan District in 2019 were collected and detected by Real-time RT-PCR.The dominant CVA6 positive samples were selected for VP1 full-length sequence amplification,sequencing and phylogenetic analysis.Results In 2019,a total of 184 sample of HFMD cases were detected,the positive rate of enterovirus was 89.67%.Of them,120 samples were positive for CVA6(72.73%),40 samples were positive for CVA16(24.24%),8 samples were CVA10(4.85%)and 3 were other enterovirus(1.82%),no EV71 was detected positive.The homology and phylogenetic analysis showed that the nucleotide and amino acid homology of VP1 gene in 24 strains of CVA6 ranged from 93.4%to 99.9%and 98.4%to 100.0%,respectively.The prevalent genotype was D3 subgenotype,and the VP1 sequences of 24 strains were distributed on D3 a.2 branch.Conclusion In 2019,CVA6 was the main pathogen of HFMD in Pingshan District,and D3 a.2 was the dominant epidemic strain.
