血清B细胞活化因子与妊娠期高血压及子痫前期的关系研究

摘　　要：目的探讨血清B细胞活化因子(BAFF)与妊娠期高血压疾病(HDP)的关系及在HDP及其病情诊断中的价值。方法选取郑州人民医院HDP患者300例(病例组),其中妊娠期高血压患者(PH组)、轻度子痫前期患者(轻度PE组)、重度子痫前期患者(重度PE组)各100例,同时收集正常单胎妊娠孕妇100例为对照组。采用酶联免疫吸附法检测各组不同妊娠阶段血清BAFF含量,比较各组BAFF水平差异并分析其与临床指标的相关性。采用ROC曲线分析BAFF对HDP的相关性。结果 (1)与对照组相比,PH组、轻度PE组及重度PE组患者妊娠各阶段BAFF水平均显著升高,且轻度PE组高于PH组,重度PE组高于轻度PE组(均P<0.05);对照组孕妇BAFF水平随妊娠进行逐渐降低,而病例组逐渐升高(P<0.05)。(2)病例组孕妇BAFF水平与分娩孕周、LDL、24 h尿蛋白等临床参数有关(P<0.05);(3)妊娠早、中、晚期BAFF水平诊断HDP效果良好,鉴别PH和PE、鉴别轻度PE和重度PE效果一般。结论血清BAFF水平与妊娠高血压及子痫前期有一定相关性,对于妊娠期高血压疾病的诊断有一定价值。Objective To investigate the relationship between serum B cell activating factor(BAFF) and hypertensive disorder of pregnancy(HDP) and the diagnostic value on HDP and its condition.Methods Totally 300 patients with HDP(Case group) were enrolled, including 100 patients with gestational hypertension(PH group), mild preeclampsia(mild PE group) and severe preeclampsia(severe PE group) in each group. 100 normal pregnant women were collected as the control group. Serum BAFF levels in each pregnancy stage were measured by enzyme-linked immunosorbent assay(ELISA), and the differences in each group were compared and the correlation between clinical parameters was analyzed. Logistic regression was used to analyze the correlation of BAFF and HDP.Results(1)Compared with the control group, the BAFF levels of each pregnancy stage in the PH group, mild PE group and severe PE group were significantly higher(P<0.05),and that in the mild PE group was higher than PH group,that in the severe PE group was higher than mild PE group(allP<0.05).The serum BAFF levels in the control group decreased as gestation proceeds, and that in case group increased as gestation proceeds;(2)The level of BAFF during pregnancy was related to the clinical parameters including gestational age, LDL and 24 h urine protein(P<0.05).(3)The performance of BAFF during pregnancy for HDP diagnostic was good and that for differential diagnosis of PH and PE, mild PE and severe PE were general.Conclusion Serum BAFF level is related to the pregnancy hypertension and preeclampsia and has certain diagnostic value for hypertensive disorder complicating pregnancy.
