摘 要:目的评价性别、吸烟、高血压、糖尿病、冠状动脉疾病(CAD)、腹主动脉瘤(AAA)家族史与AAA的相关性。方法在MEDLINE、Embase、CBM以及CNKI等中心数据库全面检索国内外公开发表的关于AAA危险因素的临床全文文献(检索年限均为各数据库建库至今)。根据纳入和排除标准由两名研究者按Cochrane系统评价方法独立选择文献提取资料评价各研究的方法学质量以及提取有效数据,应用RevMan 5.3进行Meta分析,计算合并效应指标。结果最终纳入研究文献11篇,英文文献9篇,中文文献2篇。其中流行病学调查研究8篇,病例对照研究3篇。男性、吸烟、高血压、CAD均显著增加AAA发病率(OR=2.34、2.10、1.45、1.66,P <0.01),AAA家族史虽为危险因素(OR=2.96),但95%可信期间较宽。结论男性、吸烟、高血压、CAD均为AAA的危险因素,有AAA家族史人群需谨慎。Objective To review the crrelation between gender,smoking,hypertension,diabetes,coronary artery disease(CAD),family history of abdominal aortic aneurysm(AAA)and AAA.Methods The full-text documents(published in China and other countries from beginning time of database to now)about risk for AAA from MEDLINE,Embase,CBM and CNKI.After strict screening the final enclosed documents were given Meta analysis by using RevMan 5.0 and the incorporated effect indieators were calculated.Study selection and assessment,data collection and analysis were undertaken by two reviewers independently according to the Cochrane Handbook for Systenmatic Reviews of Interventions.Meta-analyses were performend through software RevMan 5.3.Results Finally,11 research papers were selcted including 9 English literatures and 2 Chinese literatures.Among them,8 were epidemiological investigations and 3 were case-control studies.Males,smoking,hypertension,and CAD were all significantly increased the incidence of AAA,which OR was 2.34,2.10,1.45 and 1.66(P<0.01),respective.Although the AAA family history is a risk factor(OR=2.96),its 95%confidence interval was wide.Conclusion Male,smoking,hypertension,and CAD are risk factors for AAA,but the AAA family history should be cautious as a risk factor.
