祛疡清热汤治疗中重度溃疡性结肠炎疗效及对患者血清LPO与SCD-1表达的影响

摘　　要：目的:探讨祛疡清热汤对中重度溃疡性结肠炎患者血清中脂质过氧化物(LPO)与硬脂酰辅酶A脱氢酶(SCD-1)表达的影响。方法:将84例中重度溃疡性结肠炎患者作为研究对象,依照随机数字表法将患者分为观察组和对照组,每组42例。对照组给予英夫利昔单抗治疗,观察组在对照组的基础上加用祛疡清热汤治疗,对比两组患者的临床疗效及不良反应的发生情况,以及在治疗前后T细胞群、LPO及硬脂酰-LPC/油酰-LPC(SCD-1)水平的变化情况。结果:经治疗结束后,两组患者均有一定疗效(观察组90.5%与对照组73.8%,χ^2=3.977,P<0.05),且不良反应发生率较低;经治疗结束后,两组的Treg/Th17相较于治疗前明显升高,且观察组的Treg/Th17显著高于对照组,差异具有统计学意义(P<0.05);经治疗结束后,两组患者血清中LPO明显下降,硬脂酰-LPC/油酰-LPC得到升高,且观察组的LPO与硬脂酰-LPC/油酰-LPC水平显著优于对照组,差异有统计学意义(P<0.05)。结论:采用祛疡清热汤治疗中重度溃疡性结肠炎具有较好的临床疗效,可调节血清LPO与SCD-1的表达水平,增强免疫功能。Objective:To investigate the effect of Quyang Qingre decoction on the expression of LPO and SCD-1 in the serum of patients with moderate to severe ulcerative colitis.Methods:84 patients with moderate to severe ulcerative colitis were studied.The patients were divided into observation group and control group according to random number table method,42 cases in each group.The control group was treated with infliximab,and the observation group was treated with Quyang Qingre decoction on the basis of the control group.The clinical efficacy and the occurrence of adverse reactions,as well as the T cell population and LPO before and after treatment were compared between the two groups.And the change of stearoyl-LPC/oleoyl-LPC(SCD-1)levels.Results:After the treatment,both groups of patients had a certain effect(90.5%of the observation group vs 73.8%of the control group,χ^2=3.977,P<0.05),and the incidence of adverse reactions was low;after the treatment,the Tregs of the two groups Th17 was significantly higher than before treatment,and the Treg/Th17 of the observation group was significantly higher than that of the control group,the difference was statistically significant(P<0.05);after treatment,the serum LPO of the two groups of patients decreased significantly,hard Fatty acyl-LPC/oleoyl-LPC was elevated,and the LPO and stearoyl-LPC/oleoyl-LPC levels of the observation group were significantly better than the control group,reaching a statistical difference(P<0.05).Conclusion:The use of Quyang Qingre decoction has a good clinical effect on the treatment of moderate to severe ulcerative colitis.It can regulate the expression levels of serum LPO and SCD-1,enhance immune function,and is worthy of clinical promotion.
