国产与进口伏立康唑用于肾移植术后肺部真菌感染患者的疗效及安全性比较

摘　　要：目的:探讨国产伏立康唑与进口伏立康唑用于肾移植术后肺部真菌感染患者的疗效及安全性。方法:回顾性选取2018年9月至2020年5月中国人民解放军总医院第八医学中心肾移植术后可疑或确诊为肺部真菌感染的患者60例,按照治疗药物的不同分为国产伏立康唑组和进口伏立康唑组,每组30例。比较两组患者的临床疗效、不良反应发生情况、平均治疗时间和治疗费用的差异。结果:进口伏立康唑组患者的治疗有效率为83.3%(25/30),明显高于国产伏立康唑组的63.2%(19/30),差异有统计学意义(χ^2=5.08,P=0.024)。进口伏立康唑组、国产伏立康唑组患者的不良反应发生率分别为13.3%(4/30)、20.0%(6/30),差异无统计学意义(χ^2=0.48,P=0.49)。进口伏立康唑组患者的平均治疗时间为(19.35±5.47)d,平均治疗费用为(35710.21±4762.18)元;国产伏立康唑组患者的平均治疗时间为(21.47±6.25)d,平均治疗费用为(12248.05±690.01)元;进口伏立康唑组患者的治疗费用明显高于国产伏立康唑组,差异有统计学意义(t=29.03,P<0.001)。结论:对肾移植术后肺部真菌感染的患者,进口伏立康唑在疗效上优于国产药品,安全性与国产药品未见显著性差异,但治疗费用明显高于国产药品。OBJECTIVE:To probe into the efficacy of safety of domestic and imported voriconazole in the treatment of pulmonary fungal infection after renal transplantation.METHODS:Totally 60 patients with suspected or confirmed pulmonary fungal infection after renal transplantation admitted into the Eighth Medical Center of Chinese PLA General Hospital from Sept.2018 to May 2020 were retrospectively selected and divided into domestic voriconazole group and imported voriconazole group according to different drug therapy,with 30 cases in each group.Differences in clinical efficacy,incidences of adverse drug reactions,average treatment time and treatment cost were compared between two groups.RESULTS:The total effect of imported voriconazole group was 83.3%(25/30),which was significantly higher than that of domestic voriconazole group(63.2%,19/30),with statistically significant difference(χ^2=5.08,P=0.024).The incidences of adverse drug reactions of domestic voriconazole group and imported voriconazole group were respectively 13.3%(4/30)and 20.0%(6/30),the difference had no statistical significance(χ^2=0.48,P=0.49).The average treatment time of imported voriconazole group was 19.35±5.47 days,with the average treatment cost was 35710.21±4762.18 yuan;the average treatment time of domestic voriconazole group was 21.47±6.25 days,with the average treatment cost was 12248.05±690.01 yuan;the treatment cost of imported voriconazole group was significantly higher than that of domestic voriconazole group,with statistically significant difference(t=29.03,P<0.001).CONCLUSIONS:For patients with pulmonary fungal infection after renal transplantation,imported voriconazole has better efficacy than domestic voriconazole,while there is no significant difference in safety between two drugs,but the treatment cost of imported voriconazole is significantly higher than that of domestic voriconazole.
