参附汤合葶苈大枣泻肺汤治疗慢性充血性心力衰竭阳虚水泛证的临床观察

摘　　要：目的:探讨参附汤合葶苈大枣泻肺汤治疗慢性充血性心力衰竭阳虚水泛证的临床效果。方法:将50例慢性心力衰竭(Chronic Heart Failure,CHF)阳虚水泛证患者随机分成对照组与观察组,每组25例。对照组给予西医常规抗心衰基础治疗,观察组在此基础上给予参附汤合葶苈大枣泻肺汤治疗,两组患者均接受8周的治疗。观察并比较两组患者治疗前后中医证候总积分、中医证候疗效、N-末端脑钠肽前体(NT-pro BNP)、6 min步行试验等情况。结果:治疗后,对照组与观察组的中医症状积分均有降低趋势(P<0.05),观察组下降幅度明显大于对照组(P<0.05);观察组的中医证候总有效率显著高于对照组;治疗后,两组患者血清NT-pro BNP水平显著下降(P<0.05),观察组下降幅度显著大于对照组(P<0.05),两组患者6 min步行距离均有提高,观察组提高幅度显著大于对照组(P<0.05)。结论:结论参附汤合葶苈大枣泻肺汤治疗CHF疗效显著,可有效改善心功能,减轻其相关心衰症状,调节NT-pro BNP水平,利于患者预后。Objective:To explore the clinical effects of the Shenfu decoction(参附汤)and the Tingli Dazao Xiefei decoction(葶苈大枣泻肺汤)on chronic congestive heart failure with the Yang Xu Shuifan symptom(阳虚水泛证).Methods:50 patients were randomly divided into the control group and observation group,25 cases in each group.The control group was given basic Western medicine,and the observation group was given the Shenfu decoction and the Tingli Dazao Xiefei decoction more.Results:After treatment,the TCM symptom scores in the control group and the observation group had a decreasing trend(P<0.05),and the decline in the observation group was significantly greater than those in the control group(P<0.05).The total effective rate of TCM syndromes in the observation group was significantly higher than the control group.The serum NT-pro BNP levels in two groups were decreased significantly(P<0.05),and the decrease in the observation group was significantly greater than that in the control group(P<0.05).The 6-minute walking distance in two groups were increased,and the increase in the observation group was significantly larger than that in the control group(P<0.05).Conclusion:The Shenfu decoction and the Tingli Dazao Xiefei decoction have significant curative effects on CHF,which can effectively improve heart function,relieve the symptoms of heart failure,regulate NT-pro BNP level,and benefit the prognosis of patients.
