摘  要:为建立中药治疗鸭病毒性肝炎临床试验研究的系统评价,采用Meta分析方法,按照Cochrane手册指导纳入已发表的文献并以jadad量表进行评价,检索时间为从建库起至2020年1月,采用Stata 15.0软件以死亡率为效果指标进行Meta分析。结果显示:研究共纳入20篇文献,8 476例患鸭中分别包含治疗组4 133例及对照组4 343例,试验组均采用中药治疗,对照组分别采用西药治疗、抗体治疗和中药预防;随机效应模型表明中药治疗鸭病毒性肝炎的总死亡率显著低于西药治疗组(P<0.01);固定效应模型分析显示,中药治疗组死亡率显著高于抗体治疗组(P<0.05),极显著高于中药预防组(P<0.01);发表偏倚分析结果表明,所采用的文献存在一定发表偏倚性。结果提示在雏鸭幼龄时可以服用中药预防鸭病毒性肝炎,发病时可采取抗体治疗或中药治疗降低死亡率。In order to establish a systematic review of clinical trials on mortality of duck viral hepatitis in Chinese herbal medicine, a Meta-analysis method was adopted. The published literature was selected based on the guidance of the Cochrane system instruction and the jadad scale, and the timeframe was from the establishment of the database to January 2020. Stata 15.0 software was used to perform meta-analysis with mortality as an effect indicator. The results showed that a total of 20 articles were selected in the study. The 8 476 cases of ducks included 4 133 cases in the treatment group and 4 343 cases in the control group. The experimental group was treated with traditional Chinese herbal medicine, and the control group was treated with antiviral western medicine, antibody treatment and Chinese herbal medicine prophylaxis. The random effects models showed that the total mortality of ducks treated with Chinese herbal medicine was significantly lower than that of the antiviral western medicine treatment group(P<0.01);The fixed effects models showed that the mortality of Chinese herbal medicine treatment group was significantly higher than that in antibody treatment group(P<0.05) and Chinese herbal medicine prophylaxis group(P<0.01). Nevertheless, the results of publication bias analysis demonstrated that the included literature had a certain publication bias. The results suggested that, in order to reduce duck mortality, efforts should be made to avoid the disease when ducklings were young, and Chinese herbal medicine could be used to prevent duck viral hepatitis, and antibody therapy or Chinese herbal medicine treatment can be considered.
