摘  要:目的探讨经皮椎体成形术(percutaneous vertebroplasty,PVP)治疗腰椎骨折中骨水泥用量与恢复状态和并发症的相关性。方法选择2015年4月至2018年4月于江汉大学附属湖北省第三人民医院行PVP的56例腰椎骨折患者为研究对象,根据骨水泥用量将其分为低剂量组(≤4 ml)和高剂量组(>4 ml),每组各28例。分析并比较两组患者术后恢复状态和并发症发生情况与骨水泥用量的相关性。结果低剂量组患者骨水泥用量[(3.11±0.41)ml]显著低于高剂量组[(6.15±1.07)ml](t=14.038,P<0.001)。术后6个月,两组患者椎体前段高度、椎体中段高度均显著大于术前(均P<0.05),Cobb角均显著小于术前(均P<0.05),且高剂量组患者椎体前段高度、椎体中段高度均显著大于低剂量组(均P<0.05),Cobb角显著小于低剂量组(P<0.05);Pearson相关性分析显示,骨水泥用量与椎体前段高度、椎体中段高度均呈正相关(r=0.656,0.569,均P<0.05),与Cobb角呈负相关(r=-0.345,P<0.05)。低剂量组患者并发症总发生率显著低于高剂量组(P<0.05);Pearson相关性分析显示,骨水泥用量与并发症发生率呈正相关(r=0.271,P<0.05)。结论PVP治疗腰椎骨折的骨水泥用量与恢复状态、并发症发生情况相关,在确保患者恢复状态的情况下,适当减少骨水泥用量有利于减少并发症的发生。Objective To investigate the correlation between the amount of bone cement used in percutaneous vertebroplasty(PVP)for lumbar fracture and recovery status and complications.Method Fifty-six patients with lumbar vertebral fracture who underwent PVP in the Third People's Hospital of Hubei Province,Jianghan University from April 2015 to April 2018 were selected as subjects.According to the amount of bone cement,they were divided into low dose group(≤4 ml)and high dose group(>4 ml),28 cases in each group.The correlation between the recovery status and complications and the amount of bone cement were analyzed and compared between the two groups.Result The amount of bone cement in low dose group[(3.11±0.41)ml]was significantly lower than that in high dose group[(6.15±1.07)ml](t=14.038,P<0.001).At 6 months after operation,the height of the anterior vertebral body and the height of the middle vertebral body of the two groups of patients were significantly greater than those before operation(all P<0.05),while the Cobb angle was smaller than that before operation(all P<0.05).The height of the anterior vertebral body and the height of the middle vertebral body in high dose group were significantly greater than those in low dose group(all P<0.05),the Cobb angle was smaller than that in low dose group(P<0.05).Pearson correlation analysis showed that the amount of bone cement was positively correlated with the height of the anterior vertebral body and the height of the middle vertebral body(r=0.656,0.569,all P<0.05),and negatively correlated with the Cobb angle(r=-0.345,P<0.05).The total incidence of complications in low dose group was significantly lower than that in high dose group(P<0.05).Pearson correlation analysis showed that the amount of bone cement was positively correlated with the incidence of complications(r=0.271,P<0.05).Conclusion The amount of bone cement used in PVP treatment of lumbar vertebral fractures is related to the recovery state and the occurrence of complications.In the case of ensuring the patien
