摘 要:目的探讨与分析持续被动运动对兔膝关节炎软骨缺损模型的修复作用及机制。方法采用髌韧带内侧缺损法建立兔膝关节炎软骨缺损模型,将建模成功的24只实验新西兰大白兔按数字表法随机分为3组:对照组、尼尔雌醇组、实验组,每组8只。对照组不作处理,尼尔雌醇组给予灌胃尼尔雌醇治疗,实验组给予持续被动运动治疗,2 h/d,3组均治疗观察12周。在术前1 d与术后12周记录所有实验兔的体重。采用双能X线骨密度仪测定股骨骨密度。记录最大应力、最大承载力、弹性模量、刚度等值。采用二喹啉甲酸(BCA)法测定股骨基质金属蛋白酶13(MMP-13)蛋白含量。结果3组术后12周的体重均高于术前1 d,差异有统计学意义(P<0.05);实验组、尼尔雌醇组、对照组术后12周的体重分别为(2.53±0.12)kg、(2.43±0.03)kg和(2.21±0.06)kg,尼尔雌醇组、实验组高于对照组,且实验组高于尼尔雌醇组,差异有统计学意义(P<0.05)。术后12周,实验组的股骨骨密度、股骨最大应力、最大承载力、弹性模量、刚度分别为(1.59±0.21)g/cm^2、(19.09±1.45)N/mm^2、(283.22±33.10)N、(806.33±25.01)Mpa、(1863.77±189.11)N/mm;尼尔雌醇组分别为(1.32±0.04)g/cm^2、(16.78±1.72)N/mm^2、(232.87±18.29)N、(709.87±31.55)Mpa、(1522.79±200.73)N/mm;对照组分别为(0.94±0.08)g/cm^2、(14.08±1.33)N/mm^2、(165.92±19.83)N、(368.22±22.81)Mpa、(1020.48±135.92)N/mm。术后12周,实验组与尼尔雌醇组的股骨骨密度、股骨最大应力、最大承载力、弹性模量、刚度高于对照组,且实验组高于尼尔雌醇组,差异有统计学意义(P<0.05)。实验组、尼尔雌醇组、对照组术后12周的股骨MMP-13蛋白相对表达水平分别为0.56±0.11、1.56±0.23、3.98±0.11。实验组与尼尔雌醇组的MMP-13蛋白相对表达水平低于对照组,且实验组低于尼尔雌醇组,差异有统计学意义(P<0.05)。结论持续被动运动能抑制兔膝关节炎软骨缺损�Objective To explore and analysis the repairing effect and mechanism of continuous passive exercise on rabbit knee arthritis cartilage defect model.Methods A rabbit knee arthritis cartilage defect model was established by the method of medial patellar ligament defect,and 24 experimental New Zealand white rabbits that were successfully modeled were randomly divided into three groups(control group,nilestriol group,experimental group)-8 cases in each groups by digital table method.The control group were not treated,and the nilestriol group were given intragastric administration of nilestriol,the experimental group were given continuous passive exercise therapy,and the three groups were treated for 12 weeks.The body weights of all rabbits were recorded 1 day before operation and 12 weeks after operation.Femur bone density was measured using dual-energy X-ray bone densitometer.The maximum stress,maximum bearing capacity,elastic modulus,stiffness were recorded.Femur matrix metalloproteinase13(MMP-13)protein content was determined using the quinolinic acid(BCA)method.Results The body weight of the three groups at 12 weeks after surgery was higher than that of 1 day before surgery,and the difference was statistically significant(P<0.05);the weight of the experimental group,nylestriol group,and control group at 12 weeks after surgery were(2.53±0.12)kg,(2.43±0.03)kg and(2.21±0.06)kg,the nilestriol group and the experimental group are higher than the control group,and the experimental group is higher than the nilestriol group,the difference is statistically significant(P<0.05).At 12 weeks postoperatively,the femoral bone density,maximum femoral stress,maximum bearing capacity,elastic modulus,and stiffness of the experimental group were(1.59±0.21)g/cm^2,(19.09±1.45)N/mm^2,(283.22±33.10)N,(806.33±25.01)Mpa,(1863.77±189.11)N/mm;nilestriol group were(1.32±0.04)g/cm^2,(16.78±1.72)N/mm^2,(232.87±18.29)N,(709.87±31.55)Mpa,(1522.79±200.73)N/mm;the control group were(0.94±0.08)g/cm^2,(14.08±1.33)N/mm^2,(165.92±19.83)
